Please print, stamp or type the above statement directly onto the body of the commercial or Canada Customs Invoice, or present it as a separate document. 2. If presented separately, you must provide a multi or single shipment declaration. (a) A single shipment declaration must provide a reference to an individual shipment by means of a reference number. The reference number may be an invoice, purchase order, customer order number or any piece of information that clearly links this declaration to a single shipment, for example, unique letter of credit number, master or house air or ocean waybill, or shipping date with container number. (b) A multi shipment declaration must indicate an effective start and end date of a range no greater than one year (example Jan 1 to Dec 31), which clearly identifies the goods above or on an attached list. You must provide a renewal declaration on an annual basis. 3. This single or multi shipment declaration may be photocopied, faxed, scanned or transmitted as an email attachment. 4. When the origin declaration is completed by an approved exporter within the meaning of Article 17 of Annex C, the exporter's customs authorization number shall be included. A customs authorization number is required only where the exporter is an approved exporter. A signature is not required for authorized exporters. The declaration must bear a customs authorization number or a hand written signature. 5. "Canada/EFTA preferential origin" means qualifying as originating under the rules of origin of the Free Trade Agreement between Canada and the States of the European Free Trade Association (Iceland, Liechtenstein, Norway and Switzerland). For the purposes of the bilateral agricultural Agreements, the term “Canada/EFTA” shall be replaced by “Canada/Iceland”, “Canada/Norway” or “Canada/Switzerland”, as applicable. 6. The place and date of completion of the origin declaration may be indicated elsewhere on the invoice or on another document that describes the originating products and includes the origin declaration.
