WORKING RELATIONSHIPS: Internal: Program support teams - TTW, Community Participation (CP) & Life Choices teams, Disability Employment Services (DES), Clinical Support (CST), Break Thru’s Registered Training Organisation (RTO) & Employer Engagement (EET). Break Thru Support Services - IT, Business Excellence, Financial Services, Human Resources, Corporate Services & Marketing teams. External: Employers, ADHC Contract Managers, schools, training & education providers, allied health, welfare & community service providers, Centrelink & parents, guardians, carers & business services. KEY POSITION RESPONSBILITIES: Our Clients Lead & motivate the team to ensure effective day to day service delivery. Work with the Program Manager to ensure individual & team Key Result Areas (KRA’s), DSS & ADHC contractual obligations are consistently met & exceeded. Coach program team to develop Individual Transition Plans (ITP) based on identified support/training needs to transition to a work environment & refer to appropriate internal (such as CST or RTO) or external support service. Through regular & effective case reviews, monitor the development & implementation of individualised assessments that inform the Transition to Work plan. Coordinate the planning, development & review of all program curriculums including accredited & non-accredited training, ensuring the effective provision of one on one & group based training in work based skills training. Ensure the effective provision of on-the-job & work sampling training to support & build a working lifestyle. Provide information & support people with disabilities to make their own informed decisions & where appropriate, provide advocacy support. Our People With Program Manager’s support/consultation, use performance management tools & Break Thru training resources proactively to improve the individual competencies & undertake HR decisions in a timely manner that is fair & minimises risk. Support new employees via the on-boarding process utilising the mentoring framework & Break Thru’s Induction program (including timetables, etc). Cert IV Training & Assessment (TAA). Qualifications in Employment Services/Community Services/Disability (desirable). SKILLS/ATTRIBUTES: Relevant life experience &/or demonstrated understanding of the barriers to employment experienced by people with a disability. Ability to motivate self & team to participate in marketing activities to offer employment or educational opportunities for young people. Ability to coach others in identifying & developing strategies to assist young people achieve their goals via an informed plan (ITP). Ability to undertake task analysis & develop training curriculum in a variety of life skills & work tasks. Ability to manage a high workload ensuring priorities are met at all times. Sound written & verbal communication skills.
