INTRODUCTION When people think of public health, many still think of local governmental public health agencies. But a successful approach to public health requires the identification and engagement of all partners – traditional and new – that contribute to the health the community. Consistent with this systems approach, the Chicago Department of Public Health (CDPH) looks beyond its own operations to gain a better understanding and monitor the efforts of other agencies and organizations that deliver public health services. One partner of particular importance is Chicago’s hospital system. With their large investment in health care services, these institutions play a significant role in citywide efforts to improve the health of Chicagoans. Indeed, a CDPH analysis of 2001 data revealed that while safety net providers (community health centers and publicly-operated clinics) accounted for just 10% of ambulatory primary care services that year, Chicago hospitals collectively delivered 30% of outpatient primary care. This report, Chicago Hospital Profile, 1999-2003, is the most recent product of CDPH’s Health & Health Systems Capacity Project which routinely tracks and reports on Chicago’s local health care system. The Profile is a descriptive report of changes in Chicago hospitals over a five-year period. Data are presented on general measures, selected procedures, and patient payer source. In addition to the narrative report, the Profile includes an aggregate and individual profiles of each hospital during this period. Also available through the Health & Health Systems Capacity Project are profiles of the health and health systems serving each of Chicago’s 77 formally designated communities. These profiles are available on the CDPH website ( www.cityofchicago.org/Health/profiles ) as are profiles of Chicago hospitals, as well as a select few that are suburban-based yet serve large numbers of Chicagoans. In addition to the web-based profiles, the Project periodically issues special reports on areas of interest to the public health community, policymakers, and other stakeholders. Methods: This report is based on data provided by hospitals to the Illinois Department of Public Health (IDPH) in response to the Department’s Annual Hospital Questionnaire (AHQ). The AHQ is a state survey that collects information on yearly hospital utilization, patient demographics, and medical technology. All acute care hospitals licensed in the State of Illinois are surveyed, although this report focuses only on those based in Chicago. Hospitals that treat the chronically ill, such as Chicago Shriners, and federal facilities are not surveyed. The data and analyses presented in this report reflect the responses of individual hospitals as reported to and presented by IDPH. Unless otherwise noted, the changes noted in the narrative are based on absolute numbers rather than percent changes. Overall decreases noted on indicators between 1999 and 2003 include the loss of services due to hospital closures. However, when determining the leading hospitals experiencing decreases for each indicator, the closed hospitals are not included. Section II briefly describes the broad changes in hospital operations that occurred between 1999 and 2003. Sections III to V address changes in general measures, changes in selected procedures, and changes in payor sources, respectively. Section VI summarizes key findings. Appendices include: definition of terms, hospital comparison reports for selected measures, and citywide and individual hospital profiles. II. HOSPITAL CLOSURES AND OTHER TRANSITIONS With the closing of Doctors Hospital of Hyde Park in 2000 and the 2001 closures of Columbus Hospital, Edgewater Medical Center and Advocate Ravenswood Hospital, the number of Chicago hospitals declined from 42 to 38 between 1999 and 2003. While Doctors Hospital did not respond to the 1999 Annual Hospital Questionnaire (and thus is not counted in this report), the other three hospitals accounted for 22,000 admissions that year. Also during this period, Grant Hospital became Lincoln Park Hospital while Cook County Hospital relocated and became John H. Stroger, Jr. Hospital of Cook County. CHANGES IN GENERAL MEASURES, 1999 TO 2003 Hospital Admissions Chicago hospital admissions increased slightly from 416,691 to 427,109, or 2.5%, between 1999 and 2003 (Figure 1). The greatest increase occurred between 1999 and 2000 with minor fluctuations in subsequent years. Twenty-eight hospitals saw an increase in the number of admissions. Those experiencing the greatest increases were Northwestern Memorial Hospital (+11,695 or 38.1%), Rush-Presbyterian- St. Luke’s (+4,561 or 17.2%), and Swedish Covenant (+4,065 or 32.8%). The largest decreases, in absolute numbers, were Holy Cross (-2,626 or 19.1%), Stroger Cook County (-2,462 or 10.2%) and Mercy Hospital and Medical Center (-2,060 or 12.5%). Five hospitals accounted for nearly one-third (32.8%) of 2003 hospital admissions in Chicago. Eleven hospitals reported increases in ALOS, with the greatest percent increase seen at Advocate Bethany Hospital where stays rose by 25.6% from 3.9 to 4.9 days. The next largest increases occurred at Saint Mary of Nazareth (11.8% from 5.1 to 5.7 days) and South Shore Hospital (7.8% from 5.1 to 5.5 days). This is thought to be primarily due to financial pressures on hospitals to lower lengths-of-stay. The greatest percent decreases in ALOS during this time period occurred at Schwab Rehabilitation Hospital, which due to the nature of the patients served has longer lengths of stay than general acute care hospitals. The ALOS at Schwab decreased by 31% from 23.2 to 16 days. Among acute care hospitals, notable decreases were seen at Thorek Hospital (-25% from 5.2 to 3.9 days) and Lincoln Park Hospital (formerly Grant) (-22.7% from 6.6 to 5.1 days). The five hospitals with the shortest ALOS in 2003 are reflected in Figure 4. This suggests that fewer hospitals are accounting for more of the deliveries. Indeed, in 2003 five hospitals accounted for 46% of all live births at Chicago hospitals (Figure 6). Notably, over 20% of live births occurred at Northwestern Memorial, where the number of births increased by 2,801 or 40%. Northwestern Memorial accounted for 14% of live births at Chicago hospitals in 1999. In addition to Northwestern, the most notable increases were seen at Swedish Covenant (+895 or 54.9%) and Louis Weiss (+364 or 72.9%). Page 7 Chicago Hospital Profile 1999 - 2003 Emergency Room Visits Emergency room visits decreased slightly between 1999 and 2003, from 1,128,962 to 1,112,858, a drop of only 1.4% (Figure 7). Emergency room visits increased at 21 hospitals. The greatest increases occurred at Northwestern Memorial (+11,486 or 20.3%), University of Chicago Hospital (+9,401 or 15.2%), and Jackson Park Hospital (+8,810 or 57.7%). The other public hospital, Provident, also showed a decrease in visits (-6,478 or 11.3%). Consistent with their declines in admissions and live births, decreases in emergency room visits were also seen at Holy Cross (-8,797 or 17.3%) and Michael Reese Hospital and Medical Center (-6,750 or 22.6%). As reflected in Figure 8, five hospitals accounted for just over one-third (33.5%) of emergency room visits to all Chicago hospitals in 2003. CHANGES IN PROCEDURES, 1999 TO 2003 Mammograms The number of mammograms conducted at Chicago hospitals increased substantially between 1999 and 2003, from 155,093 to 194,139, an increase of 25.2% (Figure 9). However, perhaps up to half of this increase may be attributed to Rush-Presbyterian-St. Luke´s Medical Center apparently not reporting mammograms for 1999. The hospital reported over 20,000 mammograms for each of the years between 2000 and 2003. The greatest increases (based on absolute numbers and not counting Rush) occurred at Northwestern Memorial (+11,524 or 39.8%), University of Chicago (+6,009 or 58.8%), Advocate Trinity (+2,224 or 36.5%), and Advocate Illinois Masonic (+2,224 or 21.8%). Decreases, reported by 11 hospitals, were greatest at the University of Illinois at Chicago Hospital where mammograms declined by 3,324 or 46.3%. Other decreases were seen at Mercy Hospital and Medical Center (-1.367 or 15.1%), and Holy Cross (-1,093 or 25.2%). Five hospitals accounted for over half (55.5%) of all mammograms provided at Chicago hospitals in 2003 (Figure 10). IDPH did not report surgeries for 1999. Of the 17 hospitals where the number of surgeries increased, the greatest growth was seen at Advocate Trinity (+3,247 or 63.8%), Northwestern Memorial (+2,987 or 11.9%), and the University of Chicago Hospital (+2,059 or 11.6%). Among the 17 hospitals where the number of surgeries declined, the greatest decreases occurred at Mercy Hospital and Medical Center (-3,025 or 24.1%), Mount Sinai (-3,024 or 32.0%), and the University of Illinois Hospital (-2,570 or 24.3%). The majority of hospitals showed increases in the number of CAT scans of at least 25%. The greatest increases, in absolute numbers, were seen at Northwestern Memorial (+56,329 or 142.1%), Rush-Presbyterian-St. Luke’s (+20,201 or 174.1%), and Stroger Cook County (+17,980 or 77.4%). Only three hospitals showed decreases in CAT scans between 1999 and 2003. The greatest and most notable decrease occurred at St. Bernard Hospital where CAT scans declined from 2,526 to only 474 during this period, a reduction of 81.2%. The next greatest decrease was seen at Holy Cross (-1,647 or 13%). Nearly half (49%) of all CAT scans were conducted at these five hospitals. The majority of hospitals showed increases in the number of MRI scans of at least 50%. As with most other measures, Northwestern had a dramatic increase in MRI scans, up 23,369 or 181.4% during this five-year period. The other hospitals with the largest increases were Rush-Presbyterian-St. Luke’s (+7,058 or 238.7%) and University of Chicago (+4,227 or 63.2%). Only two hospitals performed fewer MRI scans in 2003 than in 1999: Michael Reese Hospital and Medical Center (-702 or 34.9%) and Lincoln Park (formerly Grant) (-161 or 29.5%). Collectively, these five sites accounted for 59.5% of all MRI scans citywide. Notably, 30% of such procedures citywide were attributable to Northwestern Memorial Hospital, which accounted for less than 19% of MRI scans five years earlier. CHANGES IN PAYOR SOURCE, 1999 TO 2003 Percent Medicaid The percentage of hospital inpatients on Medicaid increased slightly between 1999 and 2003, from 30.2% to 32.6%, an absolute increase of 2.4% (Figure 17). Of the 24 hospitals reporting an increase in the percent of patients on Medicaid, those with the greatest absolute increase were Sacred Heart (41.0%), Thorek (27.2%), and Lincoln Park (formerly Grant) (26.3%). These three hospitals accounted for very few total admissions, collectively only 4.1% of all admissions, so these increases are relatively insignificant for the system overall, although they may be significant for the individual hospitals. Of the 14 hospitals reporting a decline in the proportion of patients covered by Medicaid, the greatest absolute decreases were seen at Jackson Park (-11.4%), Roseland (-9.0%), and Michael Reese (-8.2%). Increases were reported by 24 hospitals, with the greatest absolute increases reported by Holy Cross (48.6%), Lincoln Park (formerly Grant) (29.7%), and St. Bernard (14.7%). As with Medicaid, the hospitals reporting the greatest increases in proportion of Medicare patients accounted for very few total admissions, thus while perhaps significant for the individual institutions, these increases are relatively insignificant for the system overall. Decreases in the percent of patients on Medicare were reported by 14 hospitals, the greatest seen at Sacred Heart (-21.7%), Thorek (-14.1%), and Chicago Lakeshore (-9.8%). This reflects an absolute decrease of 2.8% from 1999 when 30.6% of inpatients had insurance. Fourteen hospitals reported an increase in patients covered by insurance. The greatest absolute increases were reported by Lincoln Park (formerly Grant) (20.8%), Michael Reese Hospital and Medical Center (16.2%), and Roseland Hospital (14.9%). Among the 24 hospitals which reported a decrease in the number of commercially insured patients, those with the greatest absolute decreases included the Rehabilitation Institute (-34.6%), Mount Sinai (-28.2%), and Mercy Hospital and Medical Center (-18.6%). SUMMARY Significant Hospital Trends: While several hospitals experienced changes in the five years between 1999 and 2003, undoubtedly the most dramatic changes occurred at Northwestern Memorial Hospital. Though it has been a major force among Chicago hospitals for many years, this dominance grew dramatically during this five-year period. Admissions were up 38%, live births by 40%, mammograms 40%, CAT Scans 142%, and MRI Scans by 181%. In addition, Northwestern Memorial has ranked number one for all volume indicators (e.g., admissions, live births, surgeries, mammograms) except for emergency room visits (though even for this measure it had the greatest increase). Among the 38 hospitals in Chicago in 2003, Northwestern accounted for 10% of admissions, 20% of live births, 21% of mammograms, 20% of CAT scans, and 30% of MRI scans. While many hospitals experienced expansion in one or more areas, the only one besides Northwestern to stand out is Swedish Covenant Hospital. Among all hospitals, its ranking went from 14th to 8th for admissions, 15th to 5th for live births, and 20th to 13th for emergency room visits. For procedures its ranking remained substantially the same. Based on total admissions, the other dominant hospitals in Chicago (following Northwestern) are Rush-Presbyterian-St Luke’s Medical Center, the University of Chicago Hospital, John H. Stroger, Jr. Hospital of Cook County, and Resurrection Medical Center. Significant volume decreases for admissions, live births, and emergency room visits have occurred at several hospitals (not including the ones that closed). The most significant decreases were seen at Holy Cross Hospital, Mercy Hospital and Medical Center, Stroger Cook County, and Methodist Hospital of Chicago. Significant Indicator Findings: As previously noted, for Chicago hospitals overall, live births declined by 2.4% from 1999 to 2003. However, the share of live births for the top five hospitals in Chicago increased from 41% to 46%. This increased concentration of live births is primarily due to the dramatic increase for Northwestern. As noted above, in 2003 Northwestern alone accounted for 20% of all live births at Chicago hospitals. The next highest was Mount Sinai Hospital, with 8% of live births. Interestingly, of the hospitals in the top 10, only Northwestern and Swedish Covenant showed significant growth in the number of live births. The rest declined or stayed substantially the same. The other notable volume indicator finding was the dramatic increase in procedures, particularly CAT and MRI scans. CAT scans increased by 53.7% and MRI scans by 79.6%. Payor Analysis: With increases in the proportion of admissions covered by Medicaid and Medicare between 1999 and 2003, these two government-funded programs accounted for nearly two-thirds of all admissions (64%) in 2003. Commercially insured patients accounted for 28% of admissions. Several hospitals had dramatically higher percentages of patients with commercial insurance in 2003 than in 1999. Page 16 Chicago Hospital Profile 1999 - 2003 Regional Analysis: A geographic analysis of hospital closings suggests a pattern that deserves some comment. Map 2 shows the distribution of open and closed acute care hospitals since 1980. Four hospitals closed during the period of this analysis, 1999 to 2003: Edgewater Medical Center, Advocate Ravenswood, Columbus Hospital, and Doctors Hospital of Hyde Park. All four were located in areas with a high concentration of existing hospitals. Based on examination of Map 1, these four closings are consistent with a trend that appears to have been going on since at least 1980 in which most closings are occurring in those areas with the highest concentration of hospitals. The net effect of this is a more even distribution of hospitals throughout the city (relative to 1980). This can be seen most clearly with the substantial decrease in the number of hospitals on the city’s south side straddling 55th Street and along the North lakefront. The main exception to this appears to be the West side. Although there have been some hospital closings in this area, notably Mary Thompson Hospital and St. Cabrini, a substantial concentration remains, even after excluding the public hospital, Stroger. However, the recent merger of St. Elizabeth’s and Saint Mary’s may indicate that further consolidation in this area is occurring. The area with the lowest concentration of hospitals is the Southwest side. This is particularly noteworthy as the only hospital in this area is Holy Cross, one of the hospitals in Chicago showing notable declines in numbers between 1999 and 2003. Though not part of this analysis, an analysis of nearby Southwest suburban hospitals might shed light on this finding. Patients that were admitted more than once during the year are counted separately for each admission. Newborns are not included Average Daily Census: The average number of staffed beds that are occupied each day. The average daily census is calculated by dividing the total inpatient days by 365 days. Average Length of Stay (ALOS): The average number of days each patients stayed. The ALOS is calculated by dividing the total inpatient days by the total admissions. Emergency Room Visits: The number of visits seeking medical assistance from the hospital’s emergency department. This number may include multiple visits from the same patients. Licensed Beds: The maximum bed capacity approved by the Illinois Health Facilities Planning Board. Inventoried beds are approved by category of service. Live Births: The annual number of surviving births delivered at the hospital. Payment Source • Charity Care: Care for which the provider does not expect to receive payment from the patient or a third-party payor. • Insurance: Care which is covered by individual or group private health insurance. • Medicaid: A jointly-funded, state and federal government, insurance program that pays for medically necessary services. Medicaid pays for medical services for children and their caretakers, pregnant women, and persons who are disabled, blind or 65 years of age or older who can demonstrate a need through income and assets standards. In Illinois, Medicaid is administered by the Department of Health Care and Family Services (previously the Department of Public Aid).
