Instructions:
1. Please call 877-712-7266 option 6 to receive your RMA number.
2. Once you have received an RMA number, please complete this form and ship material to:
Ascom Wireless Solutions Inc., ATTN: RMA Department, 598 Airport Blvd, Suite 300, Morrisville, NC 27560
NOTE 1:

Remove the following accessories (if applicable):
a. SIM Card
b. Battery
c. Swivel Belt Clip

The form must be completely filled out to assure a thorough processing of the RMA.
A copy of this form must be included in each box associated with the return shipment and the
RMA number must be clearly written on the outside of each box.
NOTE 3:

All water damaged or biohazard damaged phones will not be repaired and will be sent back at the customer’s expense.

Once the equipment has arrived in NC, and has been processed we will contact you regarding warranty or repair status.
4.
If required, your P.O. should be faxed to 919-234-2526 to authorize repairs.
