1. Rebate is valid on professional and Rapidfire spark plugs purchased or installed between 1/1/12 and 6/30/12.
2. Complete all information accurately and legibly.
3. Enclose original, dated receipt or copy of original dated receipt from a retailer or paid Repair Order from an installer with Part Number detail.
4. If purchased from a retailer,
enclose UPC blocks from the
