It is our desire that all participating companies be aware of the requirements of the Utilities Department when submitting the completed test forms. Therefore, this letter shall serve as notice of the requirements for being included on our List of Independent Test Companies, as well as the requirements for completing and submitting the annual certifications for backflow prevention. To be added to the City of North Las Vegas’ List of Independent Testing Companies, the tester or the company they are working for must submit the following documentation: Current business licenses for City of North Las Vegas, City of Las Vegas and Clark County OR a multi-jurisdiction license for the three agencies for the business they will be testing under. Proof of a minimum of $1,000,000.00 of General Liability of Insurance Current California/Nevada Section of the American Water Works Association (AWWA) general tester certification OR American Backflow Prevention Association (ABPA) tester certification. Current verification of gauge calibration. After all required documents are received, the test company will be added to the List of Independent Test Companies and the tester information will be added to our system. A copy of the approved tester list is available for viewing on the City’s website at www.cityofnorthlasvegas.com It is the responsibility of the testing company to provide a legible copy of the certification of backflow testing to our office. Reports may be submitted in person, U.S. Mail, faxed to (702) 649-9784 or e-mailed to backflowprgram@cityofnorthlasvegas.com Please note that backflow devices internal to the business (sinks, drinking fountains, soda dispensers, etc.) should be forwarded to the Southern Nevada Health District; the City of North Las Vegas maintains records only for those devices installed external to the building for domestic water, fire services, irrigation and/or common areas. Further, the City requires that: The differential flow value (DF PSID) and whether it closed tight is indicated for Check Valve #2 for all RP devices. The date and time of initial test, repairs and final tests are indicated. Tester must sign and print his/her name and indicate backflow certification number. It is also the responsibility of each company to provide to this office: copies of the annual calibration reports for tester equipment including the tester it is assigned to, and updated copies of the current licenses for all backflow testers in their employ or working under their business licenses. If you have a customer with multiple devices and the test dates are in various months, a request may be submitted in writing to have all test dates set to the same month. In order to do this, all account numbers OR backflow serial numbers must be provided. The Utilities Department retains the right to deny any request to change due dates.
