The following License to Publish ("License") must be signed and returned to the Association for Research in Vision and
Ophthalmology (“ARVO”) before a manuscript can be published. If the copyright in the contribution is owned by the author's
employer, the employer or an authorized representative must sign this form. In the event that ARVO decides not to publish the Work,
this License shall be null and void.

1. The Work shall include the manuscript and all sub‐parts therein, including figures, tables, multimedia, supplementary material,
summary statement, if submitted.
2. The undersigned warrant(s) that they are the author(s) of the aforementioned work, and that they alone possess, or they alone
could possess, all copyrights to said materials. The undersigned also warrants that the manuscript (or its essential substance) has
not been published other than as an abstract or doctorate thesis and has not been submitted for consideration elsewhere, for print,
electronic or digital publication. If the Work contains any material that is owned or controlled by a third party, Author certifies that
he/she has obtained permission on behalf of ARVO for its use (please attach evidence of this) and that the material is clearly
acknowledged within the text.
3. Ownership of the copyright shall remain with the Author(s), subject to the rights granted to ARVO in paragraphs 6 and 7. The
Author(s) shall retain the non‐exclusive right to any use of the Work, so long as the Author(s) provide(s) attribution to the place of
original publication.
4. ARVO will make the Work freely available for use by others via the Journal web site under (choose one):

5. The undersigned Author(s) shall defend, indemnify, and hold harmless ARVO, its directors, officers, affiliates, employees, and
successors from and against all damages, costs, or expenses, including reasonable attorney's fees and costs of settlement, incurred
by ARVO due to claims related to the Work, including but not limited to claims of infringement of copyright and rights of privacy and
publicity.
6. The Author(s) hereby grant(s) to ARVO an exclusive license for the full period of copyright throughout the world. In particular, this
provides ARVO with the exclusive right:
• to publish the final version of the Article in the above Journal, and to distribute it and/or to communicate it to the public, either
within the Journal, on its own, or with other related material throughout the world, in printed, electronic or any other format or
medium whether now known or hereafter devised;
• to make translations and abstracts of the Article and to distribute them to the public; to authorize or grant licenses to third parties
to do any of the above;
• to deposit copies of the Article in online archives maintained by ARVO or by third parties authorized by ARVO.
7. The Author(s) hereby grant(s) to ARVO a non-exclusive license for the full period of copyright throughout the world to create
derivative works of the Work. It is understood and agreed by Author(s) that all right, title and interest in and to such derivative works,
including the copyrights, shall vest in and belong to ARVO.
By signing this License, Author(s) warrant(s) that they have the full power to enter into this agreement. This License shall remain in
effect throughout the term of copyright in the Work and may not be revoked without the express written consent of both parties.
This License shall be governed and construed in accordance with the laws of the State of Maryland without resort to the conflicts of
laws principles thereof, and any dispute arising hereunder shall be resolved in the state and federal courts serving Montgomery
County, Maryland.
