(NRS 439.200, 447.185) 1. Any person who desires to construct a hotel or to remodel or add to an existing hotel must apply in writing to the appropriate health authority on forms furnished by the health authority, giving the name of the hotel and its location together with such other information as may be required. The application must be accompanied by plans, specifications and supporting data. 2. Plans and specifications for hotels must have been prepared by a professional engineer who is registered in this State, an architect who is registered in this State, or a licensed contractor who holds a license issued by the State Contractors’ Board in the appropriate classification and subclassification. A licensed professional engineer or a registered architect shall include his seal and signature on any plans and specifications submitted to the health authority. A licensed contractor shall include his signature on any plans and specifications submitted to the health authority. 3. The plans must be drawn to scale, include a north arrow, and be accompanied by proper specifications to permit a comprehensive engineering review. 4. The submitted plans must be approved in writing before any construction is undertaken. [Bd. of Health, Hotel and Motel Reg. Please determine the physical area in which your public accommodation facility is located or planned to be located. Once a determination has been made, contact the correct licensing agency from the list below. Call them for further requirements specific to their area of control. Depending on the nature of your business, you may need both a city and a county business license. Call the agency for advice. The agency of jurisdiction will provide a packet of information, forms, and checklists to guide you through the process. Once all items from the packet are complete, the packet must be returned to the appropriate agency for processing. Massage Business/ Estab. - Owned by Therapist If you will be performing massage in your own business/establishment, you will need to complete the LVMPD/SIS Personal History Questionnaire for Massage Therapist and submit to their office at 6830 Bermuda Road, Las Vegas, along with a check or money order for $75.00. Please note on your questionnaire that you have also applied for a Massage Business License, and that you would like the investigation completed simultaneously with the background investigation for licensure. Military Discharge Provide military discharge DD-214, if applicable. Personal Financial History Questionnaire The Las Vegas Metropolitan Police Department Personal Financial Questionnaire must be completed, with notarized signatures. Provide documents showing source and availability of your personal funds invested and loan documents, if applicable. Documentation of secured funding must be available at the time of application.
