On behalf of the Department of Licensing and Regulatory Affairs,
Michigan Public Service Commission and the Gas Operations Conference
Planning Committee, welcome to the 2014 Pipeline Safety Conference.
The pipeline safety conference is designed to educate pipeline companies on pipeline safety
regulations, regulation interpretations, compliance actions, enforcement policies, and incident
investigations. The conference helps to ensure consistent application of the pipeline safety
regulations and is required as part of the federal guidelines for states participating in the
pipeline safety program.
Please take advantage of your time at the conference to learn about new updates and ideas in
the gas industry from the many speakers that will be featured. I look forward to hearing their
words of wisdom and the insights they will share with us.
Both days of the conference will be full of valuable information from experts in their fields.

Complaint Process under the updated MISS DIG Underground
Facility Damage Prevention and Safety Act (Act 174 of 2013),
effective April 1, 2014.

Dennis Bradish is the Field Intelligence Officer assigned to Michigan for Transportation
Security Administration (TSA), Department of Homeland Security (DHS).
The Detroit Field Intelligence Officer (DTW-FIO) is responsible for all TSA intelligence
activity/operations for the State of Michigan. The DTW FIO provides 24/7 value-added
indications & warning support and intelligence analysis on transportation-related threat
information that directly impacts the U.S. transportation systems in 83 counties and covering a
territory of approximately 58,000 square miles. The DTW FIO provides support to two TSA
Federal Security Directors who oversee seventeen TSA airports to include: one CAT-X, three CAT
II, three CAT III, and ten CAT IV airports. The Detroit FIO also provides support to Amtrak, the
Detroit People Mover, major freight rail operators, the Detroit Southeastern Michigan
Intelligence Center, the Michigan Intelligence Operations Center, the High Intensity Drug
Trafficking Agency, and the Detroit JTTF.
A Michigan native, he departed Michigan after graduating from Albion College and was away
from the State for 30 years having a successful Naval career in the intelligence/cryptologic field.
After his Naval career, Dennis transferred his skills and was employed by Lockheed Martin as a
senior intelligence analyst and had assignments at Guantanamo Bay, Cuba, and Washington
D.C. In Washington Dennis became employed with TSA.
After Washington, Dennis was assigned as the Field Intelligence Officer in Atlanta, Georgia in
the initial stage of the field program and had an area of responsibility encompassing four states:
Georgia, South Carolina, Mississippi and Alabama.
Given the opportunity, Dennis accepted the position as the Field Intelligence Officer for
Michigan and returned in 2011. He still maintains a primary residence in Augusta, Georgia.

David Bull, Principal
ViaData LP
Mr. Bull is a principal in ViaData LP, publishers of WinDOT, The Pipeline Safety Encyclopedia.
WinDOT contains all federal and state regulations, over 1000 interpretations, the Gas Piping
Technology Committee Guide and all historical documents relating to the DOT regulations.
He is well known throughout the gas industry for outstanding training programs, expertise in
regulatory compliance, development of the odorization audit and evaluating manuals and
damage prevention programs.
In 2006, David spent six (6) months in Kuwait developing the KOC Pipeline Safety Regulations
based on the DOT format. In addition to the regulations, he completed a study on Safe
Distances and Class Location and wrote Public Safety Awareness and Operator Qualification
Plans.
His background includes risk assessments and compliance reviews for over 75 gas utilities for
AEGIS Insurance Services, membership on the Damage Prevention Quality Action Team (Dig
Safely), and is a staff member for the Pipeline Safety Office of Inspector Training and
Qualifications. He is currently an advisor for the SHRIMP Software developed by the American
Public Gas Association Security and Integrity Foundation.
David was Contributing Editor to Pipeline and Gas Technology magazine authoring the
monthly column Pipeline Safety Arena. He has over 39-years of experience helping pipeline
operators to assess regulatory compliance programs and manage their risk and liability
exposures.

Bruce S. Campbell, CEO
MISS DIG System, Inc.
Bruce has a bachelor of science degree in Engineering from Michigan State University.
He has testified before the Senate Energy and Technology Committee on behalf of the MISS
DIG System. Previously, declared an expert witness for excavation, construction and safety and
testified on the aforementioned topics.
He is the Treasurer of the Michigan Damage Prevention Board, a Director of One Calls of
America, Inc., a member on the One Call Systems International Data Committee, Common
Ground Alliance Stakeholder Advocacy Legislative Awareness Committee, and the Common
Ground Alliance Data Reporting and Education Committee.
Bruce participates in numerous other professional organizations including: American Public
Works Association, Auburn Hills Chamber of Commerce, Construction Association of Michigan,
Delta County Chamber of Commerce, Michigan Association of County Drain Commissioners,
Michigan Chamber of Commerce, Michigan Infrastructure and Transportation Association
(MITA), Michigan Electric and Gas Association, and the Michigan Township Association.
Special Qualifications: Formerly a member of the excavation community, where he helped
establish the Genesee Area Damage Prevention Association, promoted an excavator seat on the
MISS DIG Board of Directors, and was the first excavator elected to the MISS DIG Board.

Tom graduated with a bachelor’s degree in Civil Engineering from Michigan Technological
University.
For the past 16 years, Tom has been with Wisconsin Public Service and Integrys Energy in
natural gas distribution system engineering, construction, maintenance, standards and code
compliance.
Tom is a member of the AGA Operations Safety Regulatory Action Committee (OSRAC),
MEGA, and the Wisconsin Utility Association.

Tracy has a bachelor’s degree in Mechanical Engineering and Biological Sciences from
Michigan Technological University, and a master’s of science degree in Environmental
Engineering from Michigan State University.
Currently, Tracy is the Manager of Regulatory Compliance with Consumers Energy
Distribution, Operations, Engineering & Transmission Department. Her responsibilities include
natural gas safety and electric reliability compliance. She has over 20 years within the utility
industry with roles in generation, environmental and distribution areas.
Prior to rejoining Consumers Energy in 2011, Tracy worked as an environmental consultant
on various remediation and re-development projects in the Midwest.

Charlie Helm, Pipeline Safety Specialist (Instructor), Inspector
Training and Qualifications (TQ) Division, Pipeline and Hazardous
Materials Safety Administration (PHMSA)
U.S. Department of Transportation (DOT)
Charlie attended Louisiana State University where he earned a bachelor’s degree in Electrical
Engineering with a minor in Mechanical Engineering.
He financed College through work in Deadhorse (Prudhoe Bay), Alaska as a Roustabout and
later as an Instrument Technician. After working as a wireline Engineer, then leaving the oilfield
for about 6 years to work for the U.S. Army, including participation in DESERT STORM, Charlie
hired on as Operations Engineer with a natural gas distribution company in Alaska. In 1996
Charlie took a position with an LNG / natural gas distribution company in Alaska as Vice
President of Operations.
In 2002, Charlie hired on as a Pipeline Safety Inspector for PHMSA in the Southwest Region,
(Houston Office). He was involved in several major projects at PHMSA including his selection on
the Control Room Management Team due to Control Room and SCADA experience.
After a two year departure from PHMSA working for a specialty natural gas developer where
he designed and built LNG facilities, in 2010 Charlie returned to PHMSA in Oklahoma City,
Oklahoma.

Aaron Kantor, Manager, Public Safety Outreach and Damage
Prevention Team
Consumers Energy
Aaron holds a bachelor of arts degree from the University of Michigan, in addition to being a
graduate of the Federal Bureau of Investigation (FBI) National Academy (223rd Session) and the
Michigan State University School of Staff and Command.
He started with Consumers Energy in November of 2009 and currently is the Manager of the
Company’s Public Safety Outreach and Damage Prevention team. This team serves as the
primary point of contact for County emergency managers and public sector first responders
during significant events to ensure close coordination and operational alignment.
Aaron was previously the manager of security operations/risk management in Corporate
Security with responsibilities for security regulatory compliance at the operational level, the
Company’s business continuity program, and contractual oversight of the Company security
contractors and security infrastructure program.
Prior to joining Consumers Energy, Aaron served for 15 years with the Jackson Police
Department, where he served as the Patrol Operations Commander and the commander of the
Jackson county-wide SWAT team.

Kate Leese, Emergency Management Coordinator
Consumers Energy
(Leese - pronounced “Lee – See”)
Kate holds a Bachelor of Science degree in Biotechnology from Calvin College, Masters of
Business Administration and Master of Public Health degrees from Benedictine University
focused on Emergency Management.
Kate started with Consumers Energy in July of 2011, tasked with designing and implementing
a comprehensive Public Safety Program for the company. Currently she serves as Emergency
Management Coordinator and is responsible for the Company’s Emergency Management
Strategy which includes Business Continuity and coordination with state and local emergency
management agencies.
Prior to joining Consumers, Kate worked as a Program Director for Witt Associates, a public
safety and crisis management firm founded by former FEMA administrator James Lee Witt. In
that role, Kate managed critical infrastructure assessments of food and agriculture systems in
11 Midwest states. Her projects also included capability assessments and emergency planning
with universities, local governments, and their respective chemical and utility industry partners.
Kate is Homeland Security Exercise and Evaluation Program (HSEEP) certified and codeveloped an exercise program that was recognized by the International Association of
Emergency Managers with the 2009 Business and Industry Preparedness Award for work at
Georgetown University.

Mitch Loomis, Senior Engineer-System Integrity
Consumers Energy
Mitch holds a bachelor of science degree in Metallurgical Engineering from Michigan
Technological University in 2002, and a master’s of science degree in Engineering Management
from Kettering University in 2010.
He has held jobs in the automotive industry and the aerospace industry as a Metallurgical
Engineer prior to joining Consumer Energy in 2005. For Consumers Energy, he worked in the
Power Generation Business at the Karn/Weadock Generating Facility for nearly 6 years, where
he was responsible for environmental equipment reliability, boiler reliability, weld repairs, and
plant condition assessments.
In 2012, Mitch joined Consumers Energy Pipeline Integrity group, where his main
responsibility is pipeline remediation projects and pipe assessments. With his background in
Metallurgy, API Code, and weld procedure development, Mitch also works with their weld
committee members to develop and maintain our pipeline weld procedures.
Professionally, Mitch really enjoys and takes pride in improving processes and procedures for
the better of the Consumers Energy company and its customers. Codes, Standards, and
procedures are a constant evolution, and reviewing them and updating them is vital to our
success.

Leticia Santos-Hernandez, Pipeline Safety Specialist
(Instructor), Inspector Training and Qualifications (TQ) Division,
Pipeline and Hazardous Materials Safety Administration
(PHMSA)
U.S. Department of Transportation (DOT)
Leticia Santos-Hernandez joined the U.S. Department of Transportation, Pipeline and
Hazardous Materials Safety Administration’s Pipeline Safety Office of Training and Qualification
as a Pipeline Safety Specialist (Instructor) in December 2013.
Leticia began her career as a Pipeline Safety inspector in the Puerto Rico Public Service
Commission (PSC) in 1996. After 5 years as an inspector, Leticia served as Interim Program
Director in her last year at the PSC. During her service in the PSC she identified all the propane
jurisdictional systems in the Island, making it possible for the Commission to start bringing
operators up to compliance with the applicable regulations by offering training and PHMSA
seminars. She also assisted in the implementation of the One-call System, which is currently
operated by the Puerto Rico PSC.
From 2002 to 2005, Leticia worked for a hazardous materials transportation company in
Puerto Rico as Safety and Compliance Manager and in 2009, she moved to Florida as the

Barbara is a graduate of Michigan State University with a bachelor of science degree in
Geography, with a focus on Cartographic Studies.
She has been with DTE Energy since 1995. Her current role at DTE Energy is the
responsibility for updating and maintaining the GIS Mapping system for Gas Operations
statewide. She is the lead for GIS Strategy development and working with all DTE corporate
systems that interface or are related to the mapping of gas assets. She provides reporting and
the creation of spatial queries and maps from the GIS mapping system.
The development of GIS training curriculum and implementation for office and field users of
the mapping system are one of the components of Barbara’s position. System upgrades,
system improvements, and corporate initiatives requiring GIS support are a function of her
responsibility at DTE Energy. She supports GIS initiatives in her company, her community and
promotes educational opportunities to students and co-ops within her department at DTE
Energy.
She participates in ESRI’s Electric & Gas Users Group (EGUG) and is a past Conference Chair
and committee member. She is actively involved with the IMAGIN Organization (Improving
Michigan’s Access to Geographic Information Networks) as a member of the Education and
Conference Committees and a past Board of Director for seven (7) years.

Marc is a graduate of Michigan Technological University (MTU) with a bachelor’s degree in
Mechanical Engineering and a Master’s degree from Central Michigan University (CMU) in
General Administration.
Upon graduation from MTU in 1991, Marc began his career at MichCon as a transmission
engineer and eventually became the Manager of the Belle River Gas Storage Facility.
In 2007, Marc joined SEMCO Gas Company and is currently its Vice President of Operations
and Engineering in Port Huron, Michigan.

Committee, a member of ASTM F-17, a member of the PHMSA/NAPSR Plastic Pipe Ad Hoc
Committee, a member of the public awareness program working group (PAPWG) for PHMSA
and was a task force member of the Pipeline and Informed Planning Alliance.

Mission
The mission of the Michigan Public Service Commission is to grow Michigan's economy and
enhance the quality of life of its communities by assuring safe and reliable energy,
telecommunications, and transportation services at reasonable rates.

MPSC Divisions
The Michigan Public Service Commission is organized into eight (8) Divisions:
1. Regulatory Affairs, is primarily responsible for the drafting of Commission orders and
administrative rules, and maintaining the Commission's files and official dockets;
2. Operations & Wholesale Markets, which is primarily responsible for ensuring safe,
adequate, and reliable energy supplies;
3. Electric Reliability, is primarily responsible for implementation of the state's Renewable
and Energy Efficiency Act (PA 295) and evaluating certificate of necessity filings;
4. Regulated Energy, is primarily responsible for electric and natural gas utility ratemaking
functions;
5. Financial Analysis & Audit, is primarily responsible for accounting and audit issues,
reconciliations, financial statistics, annual reports, and administration of Michigan's gas
and electric customer choice programs;
6. Telecommunications, is primarily responsible for all aspects of the Commission's
authority related to telecommunications services;
7. Motor Carrier, is primarily responsible for motor carrier regulation and licensing; and
8. Service Quality, is primarily responsible for assisting utility customers and enforcement
of Commission rules relating to customer service.
As of December 31, 2013, the Commission had a staff of 186 full-time employees.

Operations & Wholesale Markets Division
The Operations & Wholesale Markets Division provides the Commission with technical
assistance, recommendations on electric and natural gas services, and issues necessary to
ensure that regulated electric and natural gas services are provided in a safe, reliable, and
efficient manner to enhance Michigan's competitive economic environment. Other division
responsibilities include monitoring other programs, such as energy and utility forecasts,
tracking the adequacy of the State's energy supply, researching, homeland security, and critical
infrastructure protection, as well as website operations and applications.
The division is also accountable for monitoring federal and regional electric and natural gas
issues, and recommending policy proposals at the federal and regional levels. Four sections
comprise the Operations and Wholesale Markets Division: Electric Operations, Gas Operations,
Energy Markets and Energy Data & Security.

David graduated from Michigan Technological University in 1993 with a bachelor’s degree in
Civil Engineering.
He began working for the MPSC in 1994 as an engineer, auditing natural gas utilities and
pipeline companies for compliance with the Michigan Gas Safety Standards. His responsibilities
included ensuring compliance with construction, design, operation and maintenance, and risk
based integrity management program requirements. David was appointed to Manager of the
Gas Operations section at the Commission in 2011.
The Gas Operations section has responsibilities for enforcement of the Michigan Gas Safety
Standards, underground utility damage prevention, pipeline siting cases, natural gas
production, gas distribution franchises, Technical Standards for Gas Service, natural gas storage,
and related issues in natural gas utility rate cases. The section is comprised of 12 staff;
currently 11 engineers and one (1) analyst.
David is a member of the National Association of Pipeline Safety Representatives (NAPSR)
and a member of the Staff Subcommittee on Pipeline Safety with the National Association of
Regulatory Utility Commissioners (NARUC).

Tayler graduated from Lawrence Technological University in 2012 with a
bachelor’s degree in Engineering Technology.
He has been with the MPSC since October of 2012. Prior to joining the Commission as a gas
safety engineer, he worked 10 months with the Commission in the Electric Reliability Division.
Prior to joining the MPSC, Tayler worked as a Co-Op for the Department of Transportation
where he worked inspecting bridges, gathering soil samples on the boring crew, and testing
soils and geotextiles in the central soils lab.
At the MPSC, Tayler is responsible for performing inspections on the gas operators from
Northwest Lower Michigan to Southwest Lower Michigan and Central Michigan. He lives and
works out of the Lansing area, where he resides with his wife.

The Michigan Public Service Commission is composed of three members appointed by the
Governor with the advice and consent of the Senate. Commissioners are appointed to serve
staggered six-year terms. No more than two Commissioners may represent the same political
party. One commissioner is designated as chairman by the Governor.
MEMBERS OF THE COMMISSION
John D. Quackenbush, Chairman
John D. Quackenbush was appointed by Governor Rick Snyder to serve as
Chairman of the Michigan Public Service Commission on Sept. 15, 2011.
His term ends July 2, 2017.
Prior to his appointment, Chairman Quackenbush worked for UBS Global
Asset Management for 10 years, most recently as managing director and
senior investment analyst responsible for equity research for the
transportation, utilities and coal industries in the U.S. and Canada. He previously worked as
manager of the Sprint Corporation Treasury department and Sprint Corporation Local Telecom
Division. Before that, he served in several senior financial analyst roles with the Illinois
Commerce Commission.
Chairman Quackenbush was named a Top Gun U.S. Industrials Investment Mind in 2011 by
Brendan Wood International. He earned certification as a Chartered Financial Analyst (CFA) in
1993 and is a member of the CFA Institute and CFA Society of Chicago. He previously served as
a board member of the Society of Utility and Regulatory Financial Analysts. He earned a
bachelor's degree in business economics from Calvin College and a master of business
administration degree with a concentration in finance from Michigan State University.
Chairman Quackenbush is a member of the National Association of Regulatory Utility
Commissioners and serves on several committees including the Committee on Gas, the
Committee on Consumer Affairs, and the Subcommittee on Pipeline Safety. Chairman
Quackenbush is also a member of the Advisory Council for the Gas Technology Institute.

Greg R. White, Commissioner
Greg R. White was appointed by Governor Jennifer M. Granholm to serve on
the Michigan Public Service Commission (MPSC) on December 4, 2009. His
term ends July 2, 2015.
Prior to his appointment, Commissioner White served as the Legislative
Liaison for the Michigan Public Service Commission and as Liaison for the
MPSC to the Michigan Department of Energy, Labor, & Economic Growth
(DELEG). In this dual role, he represented the MPSC before the Michigan
Legislature and the U.S. Congress on matters related primarily to energy, utilities,
telecommunications, and the motor carrier industries, and also worked between the Executive
Office of DELEG and its Bureau of Energy Systems and the MPSC on matters related to energy and
energy utilities.
Previously, Commissioner White served as an Executive Assistant to MPSC Commissioner Robert B.
Nelson and to Commissioner John C. Shea. He also served as Special Assistant to Commission
Chairman John G. Strand, focusing primarily on nuclear issues. Prior to moving into the
Commissioner's office, he served on the Commission's policy staff working on issues in all utility
sectors, primarily managing congressional, and federal relations for the Commission.
Commissioner White has spent the majority of his professional career in the areas of energy and
telecommunications policy and has worked in the fields of electricity supply, nuclear issues,
renewable energy development, and oil and gas development at the MPSC, the Michigan Energy
Administration, and the Michigan Geological Survey. He also has held the position of Associate
Director at the Institute of Public Utilities at Michigan State University.
Commissioner White has provided expert testimony on matters concerning energy supply, energy
utilities, nuclear power, and telecommunications before the U.S. Congress, U.S. Nuclear Regulatory
Commission, the U.S. Nuclear Waste Technical Review Board, the Federal Energy Regulatory
Commission, and the Michigan Legislature.
Commissioner White is a member of the National Association of Regulatory Utility Commissioners
and serves on several committees, including the Subcommittee on Nuclear Issues - Waste Disposal
(Chair), Washington Action Program (Chair), the Committee on Electricity, the Committee on Critical
Infrastructure, and the Subcommittee on Utility Marketplace Access. In addition, Commissioner
White sits on the Board of Directors of the Organization of PJM States, Inc., and is a member of the
Eastern Interconnection States Planning Council.

Sally A. Talberg was appointed by Governor Rick Snyder to the Michigan
Public Service Commission on July 3, 2013. Her term ends July 2, 2019.
Prior to her appointment, Commissioner Talberg worked in the public
and private sectors over the past 15 years with a focus on energy policy
and utility regulation. Most recently, she served as a senior consultant at
Public Sector Consultants where she conducted research and analyses
and wrote reports on energy policy issues. During her tenure at Public
Sector Consultants, she also helped establish the state's premier energy efficiency financing
organization, Michigan Saves. Commissioner Talberg previously served as an analyst at the
Michigan Public Service Commission, managed enforcement and contested cases at the
Michigan Department of Environmental Quality, and advised commissioners at the Public Utility
Commission of Texas. Her expertise includes energy issues at the state, regional, and federal
level involving electric rates, retail and wholesale market design, transmission planning and
siting, and energy efficiency.
Commissioner Talberg is a member of the National Association of Regulatory Utility
Commissioners and serves on the Telecommunications Committee. She also serves on the
board and executive committee of the Organization of MISO States, and represents the state
regulatory sector on the Midcontinent ISO's Advisory Committee and Planning Advisory
Committee. She was also appointed in 2014 by the Federal Communications Commission to
serve on the Federal-State Joint Board on Jurisdictional Separations.
Ms. Talberg holds a bachelor's of science degree in Environmental and Natural Resources Policy
Studies from Michigan State University and a master's degree in Public Affairs from the Lyndon
B. Johnson School of Public Affairs, University of Texas-Austin.
