FACILITATOR EVALUATION FORM Training: T68-03 Internal Auditor Course Location: Date: June 3-5, 2003 Facilitator(s): Ned Gravel Item Course Objectives: Were you given the opportunity to help define them? Were they well defined? Were they achieved? Course Content: Was the material appropriate? Complexity (1=too complex or too simpleßàPerfect=5) Was the material clear to you? Volume (1=too much or not enoughßàPerfect=5) Did the handouts fit with this training - did they help? Facilitator Methods: Did the facilitator allow sufficient discussion? Did the facilitator encourage participation? Did the facilitator help bring out new group ideas? Did the facilitator help close out discussions? Would you accept this facilitator again? Catering and Facility: Was the seminar facility appropriate for the course? Was the lunch and breaks service acceptable? Comment/Concern: • There were no writing surfaces with the chairs • Would be very beneficial to have tables to write on. It was difficult to "spread-out" with books and handouts to go through. • Material seems too little to go over for a 3day course; yet there seems to be a lot to learn. Good material but may be clarified better to reduce some searching. • Material was somewhat disorganised - some questions in the assignments were poorly worded and at times difficult to understand what was required. May have to jury-rig some temporary surfaces for participants if we wish to continue using this facility. Noted. This course is being replaced with an IRCA certified version. Page 1 of 1 Comment/Concern: • Talk about how to get management and staff receptive and proactive about internal audits so that the resources are available to implement this process. It may be a requirement but money rules in many labs. • • • • Remedial Action: This course is primarily about how to plan and conduct audits that conform to requirements. This comment deals more with the "exercise of responsibility" principle behind 17025. Perhaps we can put more emphasis on motivational requirements in a different course on "managing a quality system." Binder pages should be numbered - beginning Noted. Course should be withdrawn and to end. replaced with an IRCA certified version within Fix principals Doc six months. Change #7 on the Principles of 17025 to Noted. Both are already done. "Repeatability" Change evaluation form to Rev 1.2 Other Comments: • Upon greater deliberation, "Objectives not achieved" were identified as not relevant to the scope of the course • Ned was able to keep the course moving and interesting • Overall - good course with some comic relief. • Great course. I would recommend it to anyone. • This course has given me the skills to conduct internal audit with greater confidence. T-18 Facilitator Evaluation Form Rev 1.2 Page 1 of 1
