It is the portion of training in which qualified candidates: • Share their findings from the assessment they complete based on the scenario provided • Analyze the results of the Eligibility Summary • Discuss perspectives with multidisciplinary team members about what is important to and for the person portrayed in the scenario • Obtain strategies that will support their success in using the MnCHOICES Assessment to conduct person centered assessments and do person centered support planning Step 3 – Part 3 is designed for interaction between the qualified candidate and: • At least one other qualified candidate or certified assessor • A multidisciplinary team 2 according to Minn. Stat. §256B.0911, Subd. 3. • A MnCHOICES Mentor • A Supervisor of Assessment and Support Planning, Home and Community Based Services or similar area of responsibility Preparation Qualified Candidate MnCAT Step 3 – Part 3 is the final component of MnCAT. To begin Step 3, qualified candidates must have completed: • MnCAT Step 1 • MnCAT Step 2 • MnCAT Step 3 o MnCAT Step 3 - Part 1: Access and Navigation o MnCAT Step 3 - Part 2: Content 1 MnCHOICES Certified Assessor Training (MnCAT) A multidisciplinary team of professionals includes at a minimum a registered nurse and social worker who are Certified Assessors. 2 2 The first task of Step 3 – Part 3 asks the candidate to complete a MnCHOICES Assessment using the: • Assessment they created during MnCAT Step 3 – Part 1 • Scenario given to them by their mentor/supervisor The final portion of Step 3 – Part 3 includes several components that can be completed in any order: • Complete the online training: “What Would You Do” • Run eligibility and analyze the results • Print reports from the Assessment • Participate in a Multidisciplinary Team meeting to: o Discuss the assessment o Consider the eligibility results o Request clarification about areas about which the candidate has questions o Learn from experienced assessors • Have discussions with their supervisor, mentor and/or coach [facilitator(s)] • Complete a DHS-6791A and DHS-6791B • Complete the LTC Screening Document Domain and print the document • Pass the final test Facilitator(s) The Facilitator should: • Be knowledgeable about and proficient in accessing and using the MnCHOICES Assessment • Have experience in long-term service and support needs assessment and support planning • Be a certified assessor or have completed all the training requirements to be a certified assessor including completion of all three (3) parts of Step 3 • Have a good working knowledge about person-centered thinking and the benefits of utilizing a multidisciplinary team • Find a location for and time to hold the multidisciplinary team meeting at which the scenario will be discussed • Invite participants to the scheduled multidisciplinary team meeting • Encourage the qualified candidate(s) throughout Step 3 – Part 3 In order to meet all of these requirements, it may be necessary to use more than one facilitator during the Step 3 – Part 3 exercises. Lead Agency/Delegate Organization Allow adequate time for staff to complete and participate in the Step 3 – Part 3 exercises. Depending on your desired approach to Part 3 and how many staff need to complete it, you may want to use different options such as to have: • Qualified candidates view and complete the online training module on their own • Qualified candidates view and complete the online training module as a group • Qualified candidates view and complete the online training module as a group using an experienced facilitator. The facilitator may be a mentor, supervisor or certified assessor 3 • • • • • Qualified candidates form their own MDT to discuss the assessment and eligibility summary Qualified candidate(s) participate in a regularly scheduled MDT meeting to discuss the assessment and eligibility summary Qualified candidate(s) have 1:1 discussions with facilitator Qualified candidates discuss work flow, work aids, etc. in a facilitated group Qualified candidates have all the resources and tools necessary to complete Step 3 – Part 3 including: During the real or simulated multidisciplinary team meeting, used to discuss the assessment completed using the scenario, make sure that the: • Qualified candidate(s) bring their laptop in offline mode with the assessment completed (all question indicators are set to zero) and eligibility run • Required screening document/service agreements are completed based on the results of the service eligibility determination • Multidisciplinary team includes at least one social worker and one RN • Multidisciplinary team members are familiar with the scenario provided to the qualified candidate(s) • Multidisciplinary team has a designated leader who will facilitate the discussion based on the guidance in this document Learning Objectives for Step 3 – Part 3 MnCAT Step 3 – Part 3 participants will be able to: • Complete an assessment based on a DHS developed scenario • Have person-centered conversations with the people they assess • Run the Eligibility Summary to compare and discuss results • Ask questions of MDT members and provide consultation to others who ask questions • Complete a DHS-6791A and DHS-6791B for the scenario • Identify resources, aids and tools that will support them as they begin their role as a certified assessor Remember • • • Requirements for participating in Step 3 – Part 3 must be completed by each qualified candidate URL to take the Step 3 test is provided to candidates only after they complete all components of Step 3 Designated lead agency administrator must co-sign the certificate to indicate that the qualified candidate meets the education and experience requirements and completed all components of MnCAT 4 Online Training Module The online training module can be accessed at any time. If the lead agency prepares for a group session where multiple candidates will view the online training together, that is encouraged. However, EACH candidate MUST register to obtain credit for this portion of MnCAT. If they do not register, they will not receive credit on their transcript which is a prerequisite for taking the Step 3 – Part 3 Test to obtain their certification. In addition, they must also “view” the module using their own unique ID to have the credit register to their transcript. Interactive Learning The interactive learning of Step 3 – Part 3 is the responsibility of the lead agency to conduct. The remainder of the guide provides instructions on how to do this. Please note the certificate is signed by the Department of Human Services indicating that the online training components are complete. However, it is not valid until co-signed by a designated lead agency administrator. The administrator attests that the qualified candidate: • Had the required education to be a Certified Assessor • Had the required experience to be a Certified Assessor • Completed all MnCAT requirements as they relate to Step 3 – Part 3 After the Final Test Don’t assume the new Certified Assessor is able to work on their own. It will take them several months to: • Become comfortable using MnCHOICES to document conversations • Understand the relationship between the responses entered and the Eligibility Summary • Have a command of work flow, eligibility requirements, screening documents and service agreements The list above is a small inventory of everything the new Assessor must master. It is extremely important that they have a supervisor, mentor and/or coach who will support them now and into the 6 future. Ongoing support and coaching promotes a professional and consistent practice across each agency and statewide. 7
