Returning your form Check that all required questions are answered and that the form is signed and dated. You can only apply if: • you already have a child support assessment or child support agreement in place • you are the person entitled to receive child support at the time you make the application and/or you are the parent where it is an application for a relevant dependent child • your child will turn 18 years of age on or before the last day of the secondary school year • your child is at least 17 years of age now, and • your child will be in full-time secondary education when he or she turns 18 years of age. An agreement is a written document, signed by both parents, setting out how much child support should be paid. If you are applying for continuation of your agreement, the other parent may also need to sign this form. If your child support assessment started with a child support agreement and the agreement states that child support is to end when the child turns 18 years of age, the child support assessment cannot be extended.
