Sillabus Entry for IGCSE / AS & A Level, LSP Checkpoint October November 2011
Please complete the attached form.
Return it with the correct amount (R650.00 per syllabus)(Checkpoint R350.00) to the CIE Office at Alma
Mater Akademie no later than the 20th of July 2011.
Please note that all subjects with a practical component have an additional fee of R650 per practical.
Should a candidate enter for the alternative to practical this fee does not apply. Please take care in
completion of this form as no refunds will be done on subjects withdrawn after the deadline on the
16th August. A late fee of R150 per syllabus is payable for all amendments to entries after the 16th of
August 2011.
A parents signature are required after each subject enrolled for.
An AS/ A Level entry fee of R150 is payable per candidate for AS/A level subjects.
Please attach a certified copy of the candidates Passport /ID and 2 recent photographs. Alma Mater
Akademie will allocate a CIE number to the candidate upon receiving these documents. This number
must be indicated on all exam papers and correspondence with the school.
