The McDuffie County Board of Commissioners is soliciting applications from General Contractors and
Subcontractors for prequalification to furnish labor, materials and equipment necessary to complete the work on
the McDuffie County Law Enforcement Center Upgrades project. The referenced project is anticipated to be bid
as a single contract for construction during 2015.
General Contractors must be pre-qualified under this application in order to bid the project.
Roofing Subcontractors must be pre-qualified under this application in order to bid the project.
HVAC Subcontractors must be pre-qualified under this application in order to bid the project.
Security Electronics Subcontractors must be pre-qualified under this application in order to bid the project.
Special Conditions:
McDuffie County Board of Commissioners is requiring mandatory pre-qualification of prospective bidders.
Therefore, interested applicants shall obtain a Pre-Qualification Application from the office of Precision
Planning, Inc. Contact Kathy Barker by email at kbarker@ppi.us to obtain the Pre-Qualification Application.
The Completed Pre-Qualification Application must be emailed to Kathy Barker (kbarker@ppi.us) no
later than 12:00 PM local time on Friday, October 9, 2015.
Do not mail application to McDuffie County Board of Commissioners or Sheriff Department.
Pre-qualification applications will be evaluated based on established criteria listed in the application. All
qualified and disqualified applicants will be notified in writing. The term of the pre-qualification shall be
effective upon issuance of notices and will expire upon award of the referenced project by the McDuffie County
Board of Commissioners. All pre-qualified contractors shall be provided notice of bids on the referenced
project.
Submitted applications will require a minimum two (2) week review and approval process beginning from the
due date of the prequalification application. Questions regarding pre-qualification applications should be
submitted in writing via email to kbarker@ppi.us no later than 12:00 PM local time on Friday, October 2,
2015.
Questions shall not be directed to any employee or department of the McDuffie County Board of
Commissioners or Sheriff Department.
Applicant shall not attach information in lieu of completion of the application document. All information
requested by the County shall be provided within the submitted application package. Additional pages may be
attached if more space is required, or as otherwise instructed within the application document. All additional
pages should be clearly labeled with applicant’s name, section, name item number and page number.

Note: McDuffie County reserves the right to approve of all personnel provided by the Contractor including but
not limited to Project Managers, Project Superintendents and subcontractors for any portion of work resulting
from this pre-qualification process.
Project Description
The Project consists of re-roofing the entire building (approximately 48,000 s.f. of roof area), replacing HVAC
rooftop equipment, installing new cooling equipment at E911 equipment room and replacing the security
electronic controls system including replacement of associated countertops/base cabinets at new security
electronic controls system equipment.
Contractor shall furnish labor, materials and equipment necessary to complete the Work on time and in
accordance with the design and contract specifications.
I.
INSTRUCTIONS
A.
Applicant SHALL NOT attach information in lieu of completion of the application
document. All information requested by the County shall be provided within the submitted
application package. Additional pages may be attached if more space is required or as
otherwise instructed within the application document. All additional pages shall be clearly
labeled with the applicant’s name, section, name item number, and page number.
B.
Applicant’s qualification will be determined based upon the information presented. All
questions must be answered in full, without exception. Failure to do so may result in the
applicant being deemed non-responsive and therefore may not be considered for
qualification.
C.
Only complete and accurate information shall be provided by the Applicant. The Applicant
hereby warrants that, to the best of its knowledge and belief, the responses contained herein
are true, accurate, and complete. The Applicant also acknowledges that the Owner is relying
on the truth and accuracy of the responses contained herein. If it is later discovered that any
material information given in response to a question was provided by the Applicant, knowing
it was false, it shall constitute grounds for immediate disqualification and termination or
rescission by the owner of any subsequent agreement between the Owner and the Applicant.
The Owner shall also have and retain any other remedies provided by law.
D.

II.
MINIMUM PREQUALIFICATION CRITERIA
A.
Bonding Company must be licensed to do business in Georgia, licensed to do business by the Georgia
Secretary of State, authorized to do business in Georgia by the Georgia Insurance Department; listed
in the Department of the Treasury’s Publication of Companies Holding Certificates of Authority as
Acceptable Surety on Federal Bonds and as Acceptable Reinsuring Companies and have A.M. BEST
Rating of A-10 or higher. Applicant must list name of Bond or Surety Company providing bond - not
just the name of Bonding Agent.

COMPANY EXPERIENCE – SIMILAR PROJECTS
GENERAL CONTRACTORS:
1.
Submit proof of at least Five (5) successful re-roofing/building renovation projects completed
within the last ten (10) years with a contract value of $2,500,000 and above (at today’s dollar
value - project costs will be escalated at 4% per year from the date of completion) with current
references.
2.
Submit proof of at least Two (2) successful projects that were completed for a
governmental/publicly funded type entity. (Can be separate projects from above requirement.)
3.
Submit proof of at least Two (2) successful projects that were a jail type facility or a facility
having a security component similar to jails, correctional, police or public safety. (Can be separate
projects from above requirement.)
4.
Additionally, all Applicants shall submit a current and fully completed AIA A305 form. This
form is to be submitted as an attachment. Failure to submit A305 may result in the application
being deemed non-responsive.
ROOFING SUBCONTRACTORS:
1.
Submit proof of at least Five (5) successful re-roofing projects completed within the last ten (10)
years with a minimum contract value for each project of $1,000,000 and above (at today’s dollar
value - project costs will be escalated at 4% per year from the date of completion) with current
references.
2.
Submit proof of at least Ten (10) successful re-roofing or roofing projects installing SBS roof
assemblies.
3.
Submit proof that bidder is a licensed installer by the roofing system manufacturer.
4.
Additionally, all Applicants shall submit a current and fully completed AIA A305 form. This
form is to be submitted as an attachment. Failure to submit A305 may result in the application
being deemed non-responsive.
HVAC CONTRACTORS:
1.
Submit proof of at least Five (5) successful HVAC projects completed within the last ten (10)
years with a minimum contract value for each project of $400,000 and above (at today’s dollar
value - project costs will be escalated at 4% per year from the date of completion) with current
references.
2.
Submit proof of at least Two (2) successful projects that were completed for a
governmental/publicly funded type entity. (Can be separate projects from above requirement.)
3.
Submit current workforce size of company. Do not include administrative staff or temporary staff.
4.
Additionally, all Applicants shall submit a current and fully completed AIA A305 form. This

ELECTRONIC SECURITY SYSTEM SUBCONTRACTORS:

Submit proof that the Electronic Security System Subcontractor (ESSS) has a minimum of 10
years’ experience in the field of integrated security and communication systems under the same
company name.

2.
Submit audited financial statement for the previous five (5) years.
3.
Submit name, address and telephone number of organization’s current Bonding Company and
level of bonding capacity.
4.
Submit evidence that the principle members and key personnel of the organization have a
minimum of 10 years’ experience each in successfully completing detention projects of equal
scope, quality, type and complexity to that required herein. Include resume(s) of personnel in the
employ of the organization that have at least five (5) years’ experience in the design, fabrication,
and installation of comparable systems.
5.
Submit proof that the ESSS is a UL listed panel shop manufacturer and able to apply the UL 508A
Listing mark at their facility.
6.
Submit proof that the ESSS has their own custom fabrication shop in-house and shall utilize their
own full-time employed engineers to manufacture all custom products. The manufacturer shall
have a minimum of two (2) full-time employed, Microsoft certified engineers.
7.
Provide evidence that the system integrator has had a full time BIM coordinator on staff for a
minimum of two (2) years.
8.
Submit a complete list of all completed projects similar to this project. Include the following:
a.
A description of systems included, along with a description of the level of integration.
References must include systems that utilized Video Graphic User Interfaces systems with
multi-touch technology.
b. Date completed.
c.
Contract value.
d. Owner contact and telephone number.
9. Submit a list of all projects, within the last five (5) years, in which the organization has been
involved in litigation.
10. Disclose any and all previous projects that the integrator did not complete the specified warranty
or complete the system installation in its entirety.
11. Submit proof that the system manufacturer will certify that they will provide one (1) complete
InduSoft software development package including license (issued to the owner). Include all other
independent development licenses such as but not limited to CCTV, audio, card access system I/O
servers utilized on the project. In addition, provide 40 hours of software programming instruction
to the owners designated representative. Programming instruction shall include a detailed
explanation of how to create I/O and memory tags, icon creation, event logging, create and edit
scripts, PLC modifications, and independent system programming methods (CCTV, audio, video
visitation, card access system) for all devices within the project.
12. Submit proof that the ESSS currently has staff based in the state of Georgia that will install and
service the system.
