Thunder Bay Community Health Service, Inc. will receive sealed bid proposals for construction trade
work from qualified contractors for new construction of a 6,400 SF addition at the Rogers City Services
site.
Sealed BIDS will be received by Thunder Bay Community Health Service, Inc. at the 15774 State Street,
Hillman, Michigan, until 1:00 PM, (Eastern Standard Time) June 15, 2016. All bids received after 1:00
PM will be returned to the Bidder unopened. The Owner reserves the right to reject any or all proposals,
accept a bid other than the low bid, and to waive informalities, irregularities, and/or errors in the bid
proposals, which they feel are in their own best interest.
Any questions regarding the bidding procedures, design and drawing/specification intent are to be directed

no unsolicited site visits will be allowed. Requests by Contractors for inclusion as Bidders should be

The Project will utilize separate prime contractors. All contracts for construction will be direct contracts
with the Owner. Overall administration of the Project will be the responsibility of the Construction
Manager. The Owner will award contracts following the review of bids, to separate prime contractors for
separate bid divisions or combinations of bid divisions. The bid divisions include but are not limited to:

Bid Division:
2016 ADDITION
Rough-in and Exterior Finishes
Provide all labor, materials, tools, and equipment necessary to perform the work of the specified bid
sections. The contractor must also furnish, deliver, unload, store, protect, erect and install all items
required for the satisfactory completion of the work of this Bid Division (as indicated on drawings
and associated specifications) including but not limited to exterior rough carpentry; interior wood
framing, attic stars, exterior wood windows; temporary and permanent bracing; wood nailers for
drywall; exterior siding, soffit, fascia, and trim, aluminum entrance doors.
General Notes:
1. There is no general contractor on this project. The respective trades assigned to that work by the
Bid Division description(s) will perform any work referenced to general contractors.
2. All contractors are responsible for the entire set of plans and specifications, including tables,
schedules, and notes.
3. Provide continuous housekeeping and clean-up, and proper legal off-site disposal of any debris
generated by this Bid Division’s work.
4. Any contractor who compounds a mistake by installing their product on another Contractor’s
obvious faulty work will assume responsibility for repair of said work.
5. Contractor is responsible for own dumpster(s) and all removal and disposal charges thereof. (The
Owner’s dumpsters are not to be used.)
6. Furnish, deliver, unload, store, protect, erect and install all labor, material and equipment required
to perform the work in this Bid Division.
7. Submit all insurance’s, unit pricing, schedule of values, required product data and shop drawings
(if required) within two (2) weeks of the Owner’s Notice to Commence Work.
8. Coordinate with all other Bid Divisions.
9. Provide all required layout.
10. Coordinate delivery of materials with Construction Manager (48 hours) prior to unloading.
11. The ability to begin as soon as areas of work become available.
12. All contractors are required to inspect the existing project components and are to include
all work necessary to complete a full operational system in compliance with all governing
codes.
Project Specific Notes:
1. Exterior framed walls and sheathing.
2. Cut back eaves for as required for the construction.
3. Roof trusses, framing, and sheathing.
4. Wood nailers for drywall
5. All temporary and permanent bracing
6. Frame in eaves.
7. Exterior windows, doors and hardware
8. Air infiltration barrier
9. Double 6” vinyl siding, soffit, fascia, and trim
10. Underlayment, drip edge, 30 year dimensional shingles, and vents
11. Provide, receive, store, protect, inventory, and install all described bid items.
12. Close cooperation with the Construction Manager to provide input to develop working schedules.
13. Provide for proper off-site disposal of all construction debris generated by the described work.

Bid Division:

Plumbing and Mechanical
Provide all labor, materials, tools, and equipment necessary to perform the work of the specified bid
sections. The contractor must also furnish, deliver, unload, store, protect, erect and install all items
required for the satisfactory completion of the work of this Bid Division (as indicated on drawings
and associated specifications) for all plumbing and mechanical work. Includes but not limited to gas
service, installing domestic plumbing and fixtures and mechanical systems, and sidewalk snow melt
system.
General Notes:
1. There is no general contractor on this project. The respective trades assigned to that work by the
Bid Division description(s) will perform any work referenced to general contractors.
2. All contractors are responsible for the entire set of plans and specifications, including tables,
schedules, and notes.
3. Provide continuous housekeeping and clean-up, and proper legal off-site disposal of any debris
generated by this Bid Division’s work.

7. Perform all connections between site utilities and building.
8. All heating, air conditioning, and ventilation equipment and ductwork.
9. Supply and install sidewalk snow melt system.
10. Maintain fire ratings of penetrated walls.
11. Remove spoils from site.
12. Provide all required layout to verify that no conflict occurs with other trades.
13. Provide cutting and patching as required for existing tie-ins.
14. Provide record and as-built drawings.
15. Provide temporary water as required.
16. Balance heating and cooling system.
17. Start up with Owner’s representative and Construction Manager present.
18. All final connections to any equipment or items provided by this Bid Division.

Electrical

sections. The contractor must also furnish, deliver, unload, store, protect, erect and install all items
required for the satisfactory completion of the work of this Bid Division (as indicated on drawings
and associated specifications) for all electrical work. Includes but not limited to electrical service,
installing electrical wiring, lighting systems, communications and data systems.
General Notes:

Bid Division description(s) will perform any work referenced to general contractors.
2. All contractors are responsible for the entire set of plans and specifications, including tables,
schedules, and notes.

14. All final connections to any equipment or items provided by this Bid Division.
15. Provide all data and communication wiring and terminations.

INSTRUCTIONS FOR PROPOSALS AND BID DIVISIONS
PART 1 - GENERAL
1.1
PROPOSAL FORMS
A.

Bidders shall use the copies of Proposal Forms, included in the Bidding Documents. Copies of
the Proposal Forms are acceptable.
BID DIVISION DESCRIPTIONS
A.
Each Bid division Description represents a separate, self-contained Scope of Work. Bid
Divisions are based on basic divisions of Work into which the Project has been divided for
bidding and construction.
PART 2 – PROPOSAL FORMAT
2.1 BID PROPOSALS

Bidders are required to use the forms provided by the Owner

Alternate pricing forms (if applicable to this project).

All spaces provided on the Proposal Form(s) shall be filled in. If any space provided is not utilized
by the Bidder, that space should be filled with the notation “N/A” (Not Applicable).
D.
The Proposal Form(s) shall be filled in by typewriter or printed manually in ink.
E.
Where indicated, all sums shall be expressed in words and figures. In case of discrepancy, the
words shall govern.
F.
Bidders shall not make unsolicited notations or statements on the Proposal Form(s). Alteration
of the Proposal Form(s) is not permitted.
G.
All changes to and erasures of the Bidder’s entries shall be initialed by the signer of the Proposal.
H.
Each Proposal shall include the legal name of the Bidder and a statement regarding whether the
Bidder is a sole proprietor, a partnership, a corporation, or other type of legal entity. Proposals
submitted by a corporation shall have the state of incorporation noted, and shall have corporate
seals affixed. Any Bid submitted by an agent shall have a current Power of Attorney attached,
certifying the agent’s power to bind the Bidder.

All requested alternates shall be bid and all lines filled in or the Proposal will be considered
incomplete.

2.3 PROPOSAL FOR MULTIPLE BID DIVISIONS
A.
Each Bidder shall submit only one (1) Proposal for each Bid Division he is bidding. There is no
limit to the number of Bid Divisions a Bidder can bid.
B.
Each Bidder is required to include a separate bid for each Bid Division in order to be considered
for contract award. Spaces are provided in the Proposal Form(s) to reference multiple Proposals.
C.
Multiple Bid Proposals shall contain separate Proposal Form(s) for each Bid Division being bid.

The Bid for each Bid Division shall be independent of Bids for other Bid Divisions.
PART 3 – COMPLETION OF PROPOSAL FORMS AND SEALED BID ENVELOPE
3.1 PROPOSAL
A.
Each Bid Division shall be submitted in a separate envelope.
B.
Fill in the legal name of the Bidder, the address, and the telephone number.
C.
Fill in the name and number of the Bid Division covered by the Proposal.
D.
On the Proposal Form(s), fill in the Lump Sum Base Bid for the Bid Division. Fill in the amount
in both words and figures.
E.
Fill in the numbers and dates of all Addenda issued, received, and considered part of the Proposal.
Proposals must include acknowledgement of all Addenda issued up to the Bid Date.
F.
If Alternate Bid(s) have been requested, fill in the Lump Sum Bid for each Alternate Bid in the
space provided.
G.
Fill in the number of anticipated on-site staff.
H.
Fill in the anticipated number of days to complete the Work.
I.
Fill in the anticipated number of days needed for delivery of indicated items, beginning on the
Bid Date.
J.
Fill in the names of the manufacturers, suppliers, and/or subcontractors of indicated items.
K.
If you chose to submit Voluntary Alternates or Value Engineering Suggestions, please summarize
your suggestions and state the amount to be deducted from the Base Bid.
L.
Review the “Bid Division Responsibilities” portion of the Proposal Form.
M.
If the Proposal includes exceptions or substitutions to any part of the Bidding Documents or the
Contract Documents, state the exceptions or substitutions in writing on the Proposal Form.
N.
Fill in the Bidder’s Legal name.
