services contract with the following provider and authorize the President of the Board to

1. Patchogue-Medford Union Free School District.
F. The administration recommends that the Board of Education accept the following
donations:
1. Donation in the amount of $1,855 by the Commack Road Runners Booster Club to be
used for the purchase of a new set of pole vault standards.
2. Donation in the amount of $2,141 by the Commack Road Runners Booster Club to be
used for the purchase of a new tent and used by the Commack High School track teams.
3. Donation of an upright spinet acoustic piano by Mr. Salvatore Bisculca to the school
district.

4. Donation in the amount of $250 from Novo Nordisk, Inc. on behalf of Gregory R.
Barbero as a result of a charitable match game. The donation is for the benefit of the
Indian Hollow Primary School.
5. Donation in the amount of $2,500 from Lowe’s Corporation to be used for materials
and supplies for the organic garden at Commack High School (amount adjusted from
May 8, 2014 BOE meeting).
6. Donation of $200 by the Commack Road Runners Booster Club to be used for an
invitational by the Commack High School Girls’ Spring Track Team.
7. Donation of $2,260 from the Mandracchia/Sawmill PTA to be applied toward the
Grade 5 field trip to Camp Mariah.
8. Donation of a 20-gallon fish tank from Glavinsky/Montalbano family for the Commack
Middle School.
G. The administration recommends that the Board of Education approve the following field

A. The administration recommends that the Board of Education adopt the resolution
authorizing the issuance of not to exceed $40,000,000 Tax Anticipation Notes for the
2014-2015 fiscal year (see attached).
B. The Board of Education is requested to acknowledge receipt of the statement for
professional services rendered on non-retainer matters in the amount of $30,196.90 for the
period ending April 2014 submitted by Lamb & Barnosky, LLP.
C. The Board of Education is requested to adopt the following resolution:
BE IT RESOLVED, upon the recommendation of the Superintendent of Schools, the Board
of Education authorizes Laura Newman and Kerry Reinhardt to sign on behalf of the
Commack Union Free School District for the purpose of causing certain insurance
companies to pay the proceeds of various policies that are owned by the District to the
District and in the case of joint ownership to the named beneficiaries.
D. RESOLVED, that the Board of Education of the Commack UFSD ratifies an agreement
between the District and Employee #01651 (attached).
E. RESOLVED, that the Board of Education of the Commack UFSD ratifies an agreement
between the District and the CTA regarding the Special Education Summer School Program
(attached).

recommendations approved on the following dates: March 10, 13, 17, 18, 26, and 31, 2014;
April 2, 3, 7, 9, 21, 24, 28, and 30, 2014; May 1, 5, 7, 8, and 16, 2014.
D. The administration recommends that the Board of Education conduct a second reading for
the purposes of adoption of Board Policy #1501 – Communication Between the Community
and Board Members (attached).
E. The Board of Education is requested to adopt the following resolution:
BE IT RESOLVED, that the Board of Education of the Commack Union Free School
District shall hold a regular meeting on Tuesday, July 1, 2014, at 6 p.m. (previously
scheduled for July 3, 2014) at Hubbs Administration Center, East Northport, New York. It
is anticipated that the Board will make a motion for the purpose of adjourning into
Executive Session at this time and at 8 p.m. the Board will reconvene in Public Session for
the purpose of holding its Annual Reorganizational Meeting/Regular Business Meeting.
BE IT RESOLVED, that the Board of Education of the Commack Union Free School
District shall hold a regular meeting on Thursday, August 7, 2014 (previously scheduled for
August 21, 2014), at 6:30 p.m. at Hubbs Administration Center, East Northport, New
York. It is anticipated that the Board will make a motion for the purpose of adjourning into
Executive Session at this time and at 8 p.m. the Board will reconvene in Public Session for
the purpose of holding its Regular Business Meeting.
