In the case of a homeless household, at least one adult member must meet the program definition of disabled. OASAS programs must serve persons with a diagnosable substance use disorder as their primary; participants may have co-occurring disabilities such as mental illness and/or HIV/AIDS. The referring worker must provide documentation of homelessness and disability that fits the criteria described below. Definition of Disability -- The definition of disabled [24 CFR 582.5] that is used as the basis for determining eligibility in the S+C program is as follows: S+C : The key to the definition is determining the A person shall be considered to have a disability if such person has a physical, mental, or emotional impairment which is expected to be of long-continued and indefinite duration; substantially impedes his or her ability to live independently; and is of such nature that such ability could be improved by more suitable housing conditions. impairment is of indefinite duration & substantially impedes the person to live independently. For example, mental illness or an HIV/AIDS condition that does not impede a person’s ability to independently does not qualify as a disability in S+C A person will also be considered to have a disability if he or she has a developmental disability, which is severe and chronic. Definition and Documentation of Homelessness: In general, a person is considered homeless if, without HUD assistance, he or she would have to spent time in a homeless shelter or in a place not meant for human habitation. More specifically, an individual is considered homeless if he or she is: sleeping in an emergency shelter; Sleeping in places not meant for human habitation, such as cars, parks, sidewalks, or abandoned or condemned buildings; spending a short time (90 consecutive days or less) in a hospital or other institution, but ordinarily sleeping in the types of places mentioned above; living in transitional/supportive housing with strong documentation of having come from streets or emergency shelters. New York/New York III Singles/Category F – Homeless single adults who have completed a course of treatment or are successfully participating in treatment for a substance use disorder and are at risk of street homelessness or sheltered homelessness and who need transitional supportive housing (that may include halfway houses) to sustain sobriety and achieve independent living. Families/Category G – Chronically homeless families, or families at serious risk of becoming chronically homeless, in which the head of household suffers from a Substance Use Disorder. Head of Household may also have a disabling medical 2 program. condition and/or HIV/AIDS. HRA treats Long-term Residential Treatment Facilities as an institution, therefore if a family meets the chronically homeless or at risk of chronic homelessness upon admission to a treatment facility, that family is still eligible for applying to this housing. Upstate Permanent Supportive Housing (UPSH) PSH specifically targets a population of single adults and families in recovery who have began a course of treatment and/or recovery when they were homeless. Participants either can be in formal treatment when they enter this Housing Program, may have completed formal treatment, or have never been in such formal treatment. An individual’s path to recovery could include 12-step group participation, faith-based experiences, or any other personal recovery involvement. All eligible individuals and families that are referred to the PSH program will be placed in the program based on an assessment of their individual care needs. The screening and selection criteria will include an assessment of the individual's ability to live independently and willingness to participate in substance abuse treatment. Also, the individual should not present a danger to himself or others and the individual should not require a higher level of care than what is offered by the program. Re-Entry Housing Persons with substance abuse problems that are being released on parole to NYC and would be functionally homeless if not placed in this PSH program. ”Doubling up” in an apartment where other adults are actively using and/or engaged in criminal behavior is not an acceptable housing option. Severe overcrowding is also not an acceptable housing option. This initiative is targeted for communities in NYC only. MRT (Medicaid Redesign Team) Housing High frequency, high cost Medicaid consumers: single adults who have received inpatient hospital care for detoxification from alcohol or any other psychoactive substance, acute psychiatric hospitalization, or acute hospitalization for a medical condition such as common co-occurring conditions of chronic addiction, namely diabetes, hypertension, emphysema, or other circulatory chronic diseases; received treatment in an emergency room of a general hospital for any of the substance use, psychiatric or medical conditions identified above. High frequency Medicaid consumers” means at least two inpatient hospitalizations or at least five emergency room episodes in a 12 month period. 3 All OASAS PSH utilize HUD Income Limits to help ensure the most in need of services are reached: To refer to this program the individual’s income must not exceed 50% of the median income (very low income limits) for the locality in which he/she resides. Annual income includes payments from all sources received by members of the family who are not minors. All of the following income must be counted: the gross amount, before any payroll deductions, of wages, overtime pay, and tips; Social Security Disability Income (SSDI); Supplemental Security Income (SSI); pension; Temporary Assistance to Needy Families (TANF); public assistance; unemployment benefits; workers compensation; and interest, dividends, and subsistence payments to full-time students or veterans (See 24 CFR §813.106).
