PLEASE PRINT AND ATTACH THIS FORM ALONG WTIH YOUR OCI APPLICATION Application form duly filled in online by each applicant at https://passport.gov.in/oci/ and by clicking "Online Registration". The application must then be printed, dated and signed (signature should be within the box meant for that purpose) and submitted along with one photograph of the applicant / each applicant and with requisite documents. A family, consisting of main applicant, spouse and up to two minor children, can apply in the same form i.e. Form XIX. The application form, complete in all respects along with enclosures, should be submitted to the Consulate General of India, Vancouver, through BLS International (www.blsindiacanada.com) S.No. Documents Required 1 Duly filled OCI application form as mentioned above 2 A copy of the current Canadian/foreign Passport which has validity of at least six months; If the applicant does not intend travelling for 8 weeks, then the original Canadian passport should also be submitted. 3 Previous Indian passports (in original) - the one on which the applicant entered Canada (with the Landing Stamp at the time of entry into Canada) and the last Indian passport issued (before acquiring Canadian citizenship). 4 Landing Paper (original and photocopy(ies)) of the Applicant, and where applicable of Spouse and children/parents; 5 If the applicants are on work permit and are applying for OCI for their children, then the Indian passports of the parents and their respective work permits should be submitted. Please (X) in column below Citizenship Paper issued by the Canadian authorities and its 6 photocopy(ies). A copy of the Surrender certificate for surrender/cancellation of 7 Indian passport on acquisition of foreign citizenship The birth certificate of children mentioning the parents name on 8 it. If Foreign spouse of an Indian origin spouse is applying for OCI, 9 then along with all other supporting documents, we would also need the marriage certificate (the marriage should have subsisted for two years immediately preceding the date of OCI application) and a notarized affidavit (format of which is provided here) should be submitted. 10 One (51mm x 51mm) photograph in respect of each applicant for each applicant to be pasted on the application form. The photograph should be of good quality taken against a light coloured (NOT WHITE) background showing a shot up to shoulders with full frontal view of face. As the photograph is to be scanned for documents, the resolution of photograph should be high. Photographs that do not conform to these standards will be rejected. 11 Fee payable to Outsourcing service agent Online Registration of OCI Application FORM In order to apply for OCI, you must complete both Part A and Part B, as per the instructions below: 1. Online Registration (Using Microsoft Internet Explorer only): The OCI Application is in two parts; Part A and Part B and MUST be submitted on-line by visiting the link http://ociindia.nic.in/ociindia/OnlineOCIRegistrationG.jsp . Alternatively, the Form can also be submitted online on the Ministry of Home Affairs website www.mha.nic.in at "Online Registration" on the “OCI Scheme” page. 2. When filling Part A, you can choose the application type either as an individual applicant or family group. You will be asked to complete the rest of the details accordingly. A family of 4 (spouses and two minor children) can complete part A together. Please ensure that Part A of each applicant is put together followed by one common Part B containing relevant information of all the applicants. The supporting documents should also be arranged in the same order as Part A of each applicant. All family members applying together must reside at the same address. If a family consists of more than 2 minor children, the application for the third (and all subsequent) minor children must be completed as an individual by selecting the option for an individual applicant. The Family Group option can also be opted if only one of the parents is applying along with one or two minor children. In case your children are not minor, an independent application for each such child needs to be completed. 3. Make sure to select the relevant Consulate under whose jurisdiction your residence falls. The provinces of Saskatchewan, Alberta, British Columbia, Yukon Territory, North West Territories fall under Consulate General of India jurisdiction. 4. Before clicking on the "Submit" button, verify that the information you entered in Part A is correct and complete. 5. Click ‘Submit” button to move to Part B of form. Fill in Part B online and press ‘submit’ button. It will generate a Reference No. on the top right corner of application form. Once Part A and Part B is filled out, click 'Save'. A print window would appear. Click OK to print out the requisite application form consisting of Part A & Part B. Make sure that all lines, boxes and information on the form are present and legible. 6. In case you made an error and already clicked the ‘Submit’ button, restart the process and enter the reference number and date of birth to open the same application form and you can correct the entries. A barcode and a reference number will be assigned to you by the computer upon completing the online registration. You must retain this reference number as it will be required when you submit your application at the BLS Center. 7. One recent photograph of size 51mm X 51mm of the applicant is needs to be pasted on Part A of the application form. The photograph should be of good quality taken against a light colored (NOT WHITE) background showing a shot up to the shoulders with full frontal view of face. As the photograph is to be scanned the resolution of it should be high. Photographs that do not conform to these standards will be rejected. 8. The OCI application has multiple places for signature. The applicant should sign or put a thumb impression (left thumb for male and right thumb for female) if he/she cannot sign, inside the box under the photograph on Part A of the form. Do not place both signature and thumb impression in the same box. It should contain either the signature or thumb impression of the applicant. Use either blue or black ink. It is very important that ALL APPLICANTS SIGN Part B. 9. When applying for a minor, both the parents should sign at the bottom of the application but not at the site indicated for specimen signature of the applicant. Signatures of both parents are required even if both parents are not applying for OCI. If only one parent is available to sign for the minor, proof of custody or a death certificate of the other parent must be provided. 10. Question #20 of Part A: Please make sure to give the name and address of the employer. In case you are retired, you should give the name and address of your last employer. 11. In case the space provided is not sufficient, additional sheets may be used for providing requisite details. 12. When submitting the application, please don't include any pages containing instructions after Part B. 13. You must answer all the questions in the form. Incomplete applications will not be accepted. 14. The Consulate does not accept OCI applications directly. The OCI applications, complete in all respects, may be submitted, in person, at Indian visa & consular Service Centers managed by BLS International Services (Canada) Inc.
