A comprehensive initial assessment of your child’s communication and/or feeding skills Feedback regarding the results of assessments and a written report To be involved in planning therapy goals To acquire skills and knowledge to work with your child at home For your personal information to be respected and kept confidential at all times For your Speech Pathologist to liaise with other health care professionals/teachers where necessary and as agreed Parents/Caregivers are expected to: Attend all appointments or to advise us as early as possible if you are not able to attend your scheduled appointment (prior to 9am on the same day) To arrive on time for your appointment, allowing ample time for parking your car To be actively involved in sessions and in the planning of therapy goals To complete regular homework tasks set by your Speech Pathologist To advise the clinic of any changes in contact details (e.g. phone number & address) To manage and be responsible for your child’s behaviour and safety when attending the clinic When possible, to arrange childcare for siblings. This will enable you and your child to focus on the session. Term-On, Term-Off Policy As we are a Community Health service and have a large waiting list and client caseload, we have adopted the Term On, Term Off Policy which is common in many other Community Health Centers. This means that your child will be able to access in-clinic services for 2 terms in a school year. Your child will be given home practice activities designed by your Speech Pathologist, taking into consideration the existing goals for your child. Your child may also attend a review appointment during this time. For example: Cancellation Procedure We understand that situations can sometimes arise in which you are unable to attend your individual or groupbased therapy appointment. Please inform us of your inability to attend as soon as possible and prior to 9:00am on the day of your appointment. You may leave a message with our reception staff or on the answering machine. Please clearly state: your child's name, your Speech Pathologist’s name, the time of your appointment and the group that you were attending (where appropriate). Note: For the consideration of other adults and children, please call us to cancel if you or your child is sick. Term 1: Group/Individual sessions Term 2: Home Practice/Review Did Not Attend Procedure Term 3: Group/Individual sessions Term 4: Home Practice/Review Due to the large number of children waiting for speech pathology services, Maroondah Community Health Speech Pathology has a strict Did Not Attend policy: If you do not attend your appointment without notifying us (as per the cancellation procedure), you will be notified by letter. If you do not attend any 3 sessions without notification over the course of your child’s therapy program, your child will be automatically discharged from the service
