REGISTRATION CHECKLIST for NEW AND RETURNING (FORMER) STUDENTS NOTE: All current 8th grade students in Providence Public Schools will be provided with customized school choice forms. These forms must be turned in at the child’s current school no later than February 4, 2016. We look forward to welcoming your child to the classroom. Registration involves a few simple steps. Please note that registration requires the student’s parent or guardian to visit the Student Registration & Placement Center in person (the child does not need to be present). If above not available: Notarized Affidavit of Residential Address – available on our website or at the Student Registration Center – and proof of residency in the name of the person with whom you reside and who is listed on the affidavit Gather the required documents (original or certified): Proof of parent/guardian identity (photo identification) and one: Student birth certificate identifying same parent or Court/legal document establishing guardianship or Notarized letter from a parent on the birth certificate identifying the guardian Visit www.providenceschools.org to download, print and complete the following registration forms: New Student Information Form Health History Form Student History Form Home Language Survey (The information requested on this form is necessary for the most appropriate placement for your child as required by Rhode Island Law and the Equal Educational Opportunity Act and will not be used for any other purposes) One proof of student identification Original Birth Certificate or Passport or Naturalization papers or I-94 Arrival card or Permanent Resident Card One proof of residency showing parent/guardian name and dated within 60 past days Lease or mortgage statement; Section 8 agreement; gas, electric or phone bill; paystub or bank statement Proof of required immunizations/health exam Students who have previously attended school: Grades K–8: Most recent report card. Students who have received Special Education services of any kind: Copy of IEP (Individualized Education Program) If registration is complete, and a language assessment is not required, the parent will be asked for school preference. If the home language survey indicates that a language assessment is required, an appointment will be made for the child to meet with the ELL teacher for assessment. After assessment and determination of educational program, the parent/guardian will be asked for school preference for the child.
