A link between past and present
On a summer day, Mr. John Smead's days are
peaceful. Quiet. Lovely. A June breeze drifts
through a garden outside his door. Life is good.
Mr. Smead earned this tranquility through a long
life dedicated to family, community – and country.
The WWII veteran experienced the dark sides of
life during the war, and he prevailed. He sacrificed
on foreign
fields to
wage a
war for
peace; the
peace that
he, and we,
now enjoy,
but never
take for
granted.
On this
day, those
travails of
war are

long gone,
and some forgotten, as years take some of the
rough edges off the memories. Days are spent
with at home with his family including his
daughter Linda Gibson and son-in-law Bob
Gibson, who see to his every need with devotion.
Many of his peers, his friends of the war era, have
passed; but Mr. Smead makes new friends, too.
Douglas Pinner is a friend to veterans like Mr.
Smead and many others. A veteran himself, the
retiree visits veterans through the “We Honor
Veterans” program at Visiting Nurse.
On this day, a Thursday, Mr. Pinner arrives for a
visit. Chit-chat turns to special conversation about
old times, as Doug, a retired manufacturing CEO,
goes back in time with Mr. Smead. Doug is a
younger man, but he relates to Mr. Smead through
a common core of experience as a fellow veteran,
and that is the magic of this program. Doug has

The New Indiana POST Form
One of the first things that comes up when
we initially meet with a patient and family
to discuss Visiting Nurse services is the
individual Plan of Care. In our hospice
program we deal exclusively with patients
who are nearing the last months/ weeks/
days of their lives and we talk with them
about what they envision for the time they

have remaining. As we discuss their goals
of care, they often talk about not wanting to be in pain or suffering
from other distressing symptoms. They want to be able to spend
quality time with friends and family, doing those things that are
important to them. They may share their worries about dying alone
or their fears about the dying process itself. Some patients come to
us more prepared to have these types of conversations while others
are not able to talk about it at all. The primary factor in the ease in
which this conversation unfolds is the amount of Advance Care
Planning that has already taken place before we ever meet them.
Those who have taken the time to engage their families in
discussion about their wishes and choices for future healthcare are
better prepared for these conversations. Some of them will have
expressed their wishes in an Advance Directive through a document
like a Living Will and/or may have appointed a Healthcare
Representative to assure their wishes are made known when they
can no longer speak for themselves.
I wish I could say the majority of the patients who enter our
agency's programs are prepared and have stated their wishes and
even documented this in an Advance Directive, but unfortunately
that is usually not the case. It is human nature to put off the
inevitable as we go about the business of living our lives. Often, by
the time the patient elects hospice or palliative care, they are very ill
and unable to have those types of conversations. It is often left for
their family to sort it out and make sure everyone is on the same
page. We do our best to work through the process, addressing it on

the first visit and engaging members of the hospice and palliative
care team to follow through and help develop a plan that meets the
goals of care that the patient would choose.
I am pleased to report the Indiana legislature recently adopted a
new Advance Care Planning document, a physician's order called
POST: Physician Orders for Scope of Treatment that became
effective in July of 2013. The POST – printed on bright pink paper
– is designed to indicate the patient's preferences for treatment
near the end of life such as code status (whether or not to initiate
CPR), IV fluids, tube feedings, antibiotics, etc. The POST is different
from other Advance Directives in that: (1) It only applies to
patients who are in an advanced state of progressive illness
(generally a prognosis of one year or less);
(2) Whereas most Advance Directives are preferences expressed by
any adult, POST is an actual physician's order; (3) Most Advance
Directives are limited to the location in which they were created, but
POST is portable – it is a physician's order that follows the patient
across all healthcare
settings. It also allows for
appointment of a Health
Care Representative on
the POST form itself.

From Pearl Harbor to Hospice Home
A lifelong journey of service and valor
Gene Glosson was a
very young man living
in Richmond, Indiana
when he enlisted in
the Navy, one year
and a day before Pearl
Harbor (December 6,
1940). At that
moment, who knew
what would lie ahead
for this enthusiastic
young man.
Gene was a gunner's
mate, first class on the
USS Nevada, stationed

Hawaii. His younger brother was also serving on the same ship; both
survived the Japanese attack on December 7. The USS Nevada was
the only battleship that got underway and was later beached. Gene
later served on the USS Gunnell under Sen. John McCain's father,
John S. McCain Jr. Gene volunteered for submarine duty for the
remainder of World War II and visited many ports including Perth,
Australia, to which he later returned for a visit with his wife Teresa.
He first met her when on furlough to attend his brother's wedding to
Teresa's sister; Gene and Teresa were married for 67 years.
During the Korean conflict, Gene was called up and served his
country again, in the Navy on submarine duty.
He and Teresa settled in Fort Wayne in 1956 and raised their eight
children here. According to Gene's son John, "He didn't talk about
the war much.”
He started talking about it around the time of the 50th anniversary
of the attack on Pearl Harbor. “As a kid, all we ever heard about was
his time on leave and all the fun he had."
Nearly 15 years ago, Gene took John to visit Pearl Harbor.
"It was great getting a first-hand tour of Pearl Harbor," said John.
Gene was awarded the Purple Heart for his service. Gene was a
successful business owner, proud father, a hunter and a fisherman.
John recalled accompanying his dad
on a trip to Groton, Connecticut
for the launch of the third USS Indianapolis
and remembered how emotional it was
for all the servicemen.
He was also an active member of American Legion 296. John recalled

Mr. Glosson, age 91, died peacefully in Hospice Home on February
13, 2014, with his family by his side.

this is why I do this," says Doug Pinner. "It's rewarding like
nothing else."
It's not just two former soldiers swapping war stories; it's
much more. It's not just about reminiscing; it's about living.
Going through scrapbooks and memorabilia, Doug reminds
Mr. Smead that a grateful nation values the sacrifices he made
long ago. The valor of a young man's deeds is repaid now, and
Mr. Smead warms to the recognition. He's proud. Mr. Pinner
has a special way with veterans, which makes him so valued
as a volunteer, says Ann Blue, Volunteer Coordinator for
Visiting Nurse. "He is a wonderful example of what our
volunteers mean to our patients and families," she says. "They
all have so much to give, each in their own way, and Doug
Pinner is a special guy to an awful lot of Hospice families."
One Final Salute….Visiting Nurse is grateful to serve our Veterans
Through the "We Honor Veteran's" program, Visiting Nurse is committed to attending to the unique needs
of our veterans and their families at the end-of-life. We are humbled and grateful to be of service to our
veterans who have given so much of themselves during their lifetime of service to our country. Thank you
to the families of John Smead and Gene Glosson for sharing these important stories of selfless service so
we won't forget this sacrifice and those who serve and those who died fighting for our freedom.

Power of the Uniform

As I so frequently do after a day of work, I stopped by the grocery store to pick up
something for dinner. While placing items in my cart, I saw a young woman
walking in my direction. Not thinking anything about it, I turned to resume
pushing my cart down the aisle. I suddenly felt a gentle tug on my uniform jacket
sleeve. Standing before me was a tearful woman. With her hand placed on the
center of her chest, she stated "I want to let you know how wonderful you all are.
You came right to our home, walked us through everything we needed to know to care for my
grandmother. All of you are so wonderful. Thank you."
Like every single person
I wanted to grab her and give her hug, but my better judgment
who works at Visiting
told me, no, not this time. I was afraid there would be two
tearful women standing in the middle of the bread aisle,

was, but what I do know is that each and everyone of us works for an agency that strives to help make
the journey from being terminally ill, to death, and then bereavement. And whether we give it much
thought or not, when we're out in uniform, we stand out like no other, and we truly are appreciated.

The committee members are working together to:
(1) Educate physicians, EMS, and the various
healthcare organizations about the POST form
and how to utilize it; (2) Educate the public about
POST – the importance of having discussions with
your physician about your wishes, and how, when
and where POST can be used. The POST Steering
Committee will be providing training later this
spring for POST facilitators who will be equipped
to spread the word about POST to health care
organizations and the community in general.
At Visiting Nurse, we have seen firsthand how
difficult it is when a patient is not given
opportunity to discuss their wishes or is too ill to
be able to do so. We have also seen the effect
this has on family members – each of whom is
trying to do their best to assure their loved one
receives the care they would have wanted, but
many times disagreeing among themselves about
just what their loved one's care should be. The
POST form addresses these concerns and is an
excellent tool to help patients, families, and those
who care for the seriously ill understand what the
individual's choices truly are.
POST fits very well within the mission of Visiting
Nurse to provide "compassionate care to alleviate
suffering and ensure quality of life for those
affected by serious illness." We thank our
community for the continued support of Visiting
