The Office of Student Financial Aid (OSFA) has been informed that all grades submitted on your behalf were not passing (W, I,
& F Grades). As a result, a calculation must be completed to determine your eligibility for financial aid. To do so, you must
submit this Confirmation of Attendance Form, signed by ALL professors for the SPRING 2013 term.
• If your form is not received by MAY 31, 2013, ALL of your federal and state funding may be returned.
• If all professors confirm attendance but no specific date is given, the calculation will be based on the midpoint
of the semester.
• If you were paid financial aid based on full time enrollment and you are not able to confirm attendance in all courses,
your aid will be adjusted to the amount of credits confirmed and the calculation will be based on the last date of
attendance for the confirmed courses.
• You have 15 days from the date of this letter to submit this form. Once all funds are returned, they cannot be
reacquired due to a failure to meet the deadline.
