I declare that I have completed this Application Form with reference to the relevant Supplemental Notes
and that all particulars given in this application and in all documents attached herewith are true and
correct. I further declare that the Critical Analysis submitted together with this application was written by
me and in my own words, except for quotations from published and unpublished sources which are clearly
indicated and acknowledged as such.
I acknowledge that I am allowed to sit for Part II Assessment in June 2016. If I fail the Part II Assessment
five times (including the first one taken together with the Part I Assessment/Written Assessment), I will be
directed to take the full Final Assessment (including the Part I Assessment and participation of the 40hour mandatory CPD Programme organized by the GPD Education Committee with not less than 80%
attendance as described in Section 8 of the GPD APC Rules and Guide June 2012 Edition).

Candidate’s application shall include the following:1) This APC6/GP/F2 Form;
2) One passport-sized photograph;
3) Logbook & Log sheet (Full record);
4) A Critical Analysis (typewritten in triplicate & a CD Rom containing the scanned copy of the Critical Analysis);
5) Record of pre-qualification structured learning (PQSL) (Full record);
6) Relevant Fee for the Part II Assessment. Please make a cheque payable to “Surveyors Services Ltd.”; and
7) A copy of the latest Chairman’s Report (Oral Assessment).
NOTE
Failure to complete all of the above may result in the application being rejected.
For any changes of Employer, Supervisor or Counsellor after submission of Form APC6/GP/F2,
candidates must inform the HKIS immediately for approval.
All applications should reach the HKIS Secretariat no later than 12:30pm, Saturday, 30 April 2016.

Supplemental Notes for Candidates
Applying for APC Part II Assessment 2016 under FORM APC6/GP/F2
1.
All Candidates should note the HKIS Rules & Guide to the Assessment of
Professional Competence (APC), General Practice Division June 2012 Edition
which could be downloaded from the HKIS website.
2.
Candidates must be a Probationer of the Institute, and pay the current and/or
outstanding subscription fee for the membership. Non compliance will result in
your application being rejected.
3.
Please complete all relevant sections of the Application Form APC6/GP/F2.
Incomplete application will not be processed.
4.
For any changes of employment, the HKIS’ approval to the changed arrangements
for professional training and experience must be obtained by making a
supplementary application on Form APC1S/GP to the HKIS Secretariat. Failure to
do so may result in your application being rejected.
5.
All Candidates are required to add the Plagiarism Statement in relation to “no
plagiarism” in their Critical Analysis immediate before their declaration
signature. Any submission without the incorporation of this statement will not be
accepted.
6.
Please note that all applications together with the relevant documents must be
submitted to the HKIS Secretariat no later than 12:30 pm, Saturday, 30 April
2016.
7.
The APC Part II Assessment 2016 for referred candidates of the APC Final
Assessment in the General Practice Division will take place between 2 June and 4
June 2016. Candidates will be informed of the exact date and time of the
assessment by mid-May 2016.
8.
Please note that if candidates fail the Part II Assessment five times (including the
first one taken together with the Part I Assessment/Written Assessment), they will
be directed to take the full Final Assessment as described in Section 8 of the GPD
APC Rules and Guide June 2012 Edition. Candidates must complete a further
minimum 90 days relevant professional training before attending the Final
Assessment.
