DCPS MAINTENANCE WORK ITEMS REQUIRING NOTIFICATION
FOR INSPECTION PER THE FLORIDA BUILDING CODE (FBC)
DCPS Maintenance Annual Permit (Issued for each Fiscal Year, July 1st to June 30th): In
lieu of an individual permit for each alteration to an existing electrical, gas, mechanical,
plumbing or interior non-structural office system(s), the building official is authorized to issue an
annual permit for Group E occupancies to facilitate routine or emergency service, repair, refurbishing, minor renovations of service systems or manufacturing equipment
installations/relocations. The building official shall be notified of changes and shall retain the
right to make inspections at the facility site as deemed necessary.
Maintenance Work Notification: The maintenance work notification shall be utilized for
alterations, equipment replacements, or new installations involving concealed work, i.e.
structural, mechanical, plumbing, and/or electrical.
1) Notification to DCPS Code Enforcement shall be made via the “Maintenance Work
Notification Form.” (Form provided separately.)
2) The work notification form shall be e-mailed for approval, prior to beginning the work, to the
DCPS Building Official and the code enforcement inspection mailbox
(codeenforcement@duvalschools.org).
3) Once the work item is approved, an “Annual Maintenance – Work Authorization” with
permit/authorization number shall be issued to the person providing notification. See
Appendix ‘A’ for sample work authorization.
4) Inspection requests shall be submitted as prescribed on page 4 of this document.
5) Work items requiring notification are provided herein.
SAP Work Orders (99 Codes): Less complex maintenance alterations, replacements or new
installations, not involving concealed work or structural modifications, should be assigned a 99
code in the SAP work order system. The building official shall review these work orders in SAP
and make inspection assignments as deemed necessary. The maintenance supervisor and
foreman will be notified by e-mail of any non-compliant items found as a result of inspection.
Building:
1) Building construction associated with MEP work notifications shall be inspected as part
of the work authorization in accordance with the Florida Building Code, for example, metal
stud framing, drywall screw inspections, sealing of penetrations in rated assemblies, concrete
work, etc.
2) Installation of exterior doors, windows, and components and cladding require
notification for inspections per FBC 110.1. Florida Product Approval and manufacturer’s
anchoring instructions are required for review and inspection.
3) Capital Crew work is permitted separately.
4) Structural work requires design by a Florida Licensed Engineer, no exceptions.

Mechanical/HVAC:
1) New installations are not covered by the DCPS Maintenance Annual Permit and require a
separate Day Labor or Contractor Permit. (FBC 102.2, 105.1, 105.1.1)
2) All HVAC/Mechanical equipment replacements (FBC105.1) require notification for
inspections per FBC 110.1 (see examples below)
a) Condensing unit replacement
b) Air handling unit replacement
c) Mini-split replacement
d) Boiler replacement
e) Chiller replacement
f) Kitchen Hood Exhaust Fan replacement
3) Exceptions (FBC 105.2): Portable mechanical/HVAC equipment or appliances, repair of
equipment, equipment parts replacement, and non-kitchen hood exhaust fan replacements do
not require notification. Emergency repair or replacement does not require prior notification,
but notification should be made the next business day (FBC 105.2.1).
4) Required Inspections (FBC 110.3):
a) Underground inspection, if applicable, prior to backfill
b) Rough-in inspection prior to cover up
c) Any required tests such as pressure tests
d) Final inspection at completion
Gas Piping & Fuel Tanks:
1) New installations are not covered by the DCPS Maintenance Annual Permit and require a
separate Day Labor or Contractor Permit. (FBC 102.2, 105.1, 105.1.1)
2) All gas piping repairs, replacements, and installations require notification for inspections per
FBC105.1 & FBC 110.1.
3) All fuel tank replacements and/or installations require notification for inspections per
FBC105.1 & FBC 110.1.
4) Exceptions (FBC 105.2): Notification for inspection is not required for portable gas fired
heating appliances and the replacement of any minor part that does not alter approval of the
equipment or make it unsafe. Emergency repair or replacement does not require prior
notification, but notification should be made the next business day (FBC 105.2.1).
5) Required Inspections (FBC 110.3):
a) Underground inspection, if applicable, after piping is installed and prior to backfill.
b) Rough piping inspection prior to cover-up and connection of fixtures and appliances.
c) Final piping inspection after piping has been completed, covered up, but prior to
connection of fixtures and appliances – includes a pressure test.
d) Gas system inspections include but are not necessarily limited to bonding of the pipe,
drip legs, sediment traps, pressure tests, pipe supports, sealing of wall/floor penetrations.

e) Final inspection upon total completion of the system after fixture and appliances are
connected.
Plumbing:
1) New installations are not covered by the DCPS Maintenance Annual Permit and require a
separate Day Labor or Contractor Permit. (FBC 102.2, 105.1, 105.1.1)
2) Water and sewer piping replacements (FBC105.1) require notification for inspections per
FBC 110.1.
3) Water heater replacements (FBC105.1) require notification for inspections per FBC 110.1.
4) Exceptions (FBC 105.2): Replacement of plumbing fixtures, repairs for leaks, and clearing
of stoppages provided such repairs do not involve replacement or rearrangement of valves,
piping systems or fixtures do not require notification. Emergency repair or replacement does
not require prior notification, but notification should be made the next business day (FBC
105.2.1).
5) Required Inspections (FBC 110.3):
a) Underground inspection after pipe is installed and prior to backfill
b) Rough-in inspection prior to cover up
c) Required tests such as pressure tests (312 of FBC, Plumbing)
d) Final inspection after completion and connection of all fixtures
Electrical:
1) New installations are not covered by the DCPS Maintenance Annual Permit and require a
separate Day Labor or Contractor Permit. (FBC 102.2, 105.1, 105.1.1)
2) All electrical equipment replacements (FBC105.1) require notification for inspection per
FBC 110.1 (see examples below):
a) Panelboards
b) Switchboards
c) Motor controllers (i.e. variable frequency drives)
d) Transformers
e) Generators
f) Exterior light poles
g) Electrical work associated with other equipment replacements such as HVAC equipment
and electric water heaters
h) Complete replacement of lighting fixtures in a space.
3) Exceptions (FBC 105.2): Replacement of switches, receptacles, lighting ballasts, individual
light fixtures and re-lamping do not require notification. Emergency repair or replacement
does not require prior notification, but notification should be made the next business day
(FBC 105.2.1).
4) Required Inspections (FBC 110.3):
a) Underground inspection after conduit or cable is installed and prior to backfill
b) Rough-in inspection prior to cover up

c) Final inspection at completion and after all final connections are made
Fire Protection:
1) Repair, replacement and new installations of automatic fire extinguishing systems are
not covered under the DCPS Annual Maintenance Permit and shall be performed by a
licensed and certified Fire Protection Contractor under separate Permit (Chapter 633 &
489, Florida Statutes; FBC 105.3.1.2). See examples below:
a) Automatic sprinkler systems, i.e. all systems covered by NFPA 13
b) Standpipe Systems
c) Kitchen Hood automatic fire extinguishing systems or suppression systems (NFPA 96)
2) New fire alarm and detection systems which cost more than $5,000 shall be designed by a
licensed Electrical Engineer and installed by at licensed Electrical/Fire Alarm contractor
under separate Permit (Chapter 633 & 489, Florida Statutes; FBC 105.3.1.2).
3) Code Enforcement should be notified when adding or deleting a fire alarm device (under
$5,000) such as adding a heat detector to a storage room.
4) Exceptions: Annual routine inspection, testing and maintenance in accordance with NFPA
72 do not require notification. Emergency repair or replacement does not require prior
notification, but notification should be made the next business day (FBC 105.2.1).
Work by Contractors:
1) Contractors performing work for DCPS Maintenance shall contact our office prior to
beginning the work to make sure all paperwork such as licenses and insurance are correct,
current and on file (Chapter 489, Florida Statutes & FBC 105.3.5). Contractors should also
contact the Security Department for ID badges.
2) Work by Contractors is not covered by the DCPS Annual Maintenance Permit; therefore,
separate Contractor Permits are required (FBC105.1).
3) There are very few exceptions to permits for Contractors, i.e. painting, ceiling tile (only)
replacement, and floor finishes.
Inspection Requests:
1) Inspection requests (FBC 110.5): It shall be the duty of the holder of the building permit or
their duly authorized agent to notify the building official when work is ready for inspection. It
shall be the duty of the permit holder to provide access to and means for inspections of such
work that are required by this code.
2) Inspection requests shall be e-mailed to DCPS Code Enforcement at
codeenforcement@duvalschools.org on the day prior to a needed inspection. The following
format shall be utilized for inspection requests (see Appendix ‘B’ for sample):
a) The required format for the SUBJECT LINE on e-mailed inspection requests is as
follows: Permit #-School Name-(Construction Trade and Type of Inspection Requested,
i.e. underground, rough-in, above ceiling, final, etc.)
b) For example: #3218011713.99AE-San Mateo Elementary-(Electrical Rough-in)

c) The SUBJECT LINE is not part of the body of the message, i.e. it is the line below the
“To” and “Cc” lines at the top of the e-mail.
d) The body of the message will be more descriptive indicating the exact location of the
work, such as 2nd Floor in Wing A. Inclusion of a contact phone number in the body is
also helpful.
e) After the inspection takes place, the results shall be e-mailed back by the assigned
Inspector and shall be filed in Code Enforcement’s “Microsoft Share Point” database
under the appropriate Maintenance Station.
3) A request for FINAL INSPECTION is required in order to close out the Maintenance Work
Item.

Sample Annual Permit Work Authorization

105.1.1 Annual facility permit. In lieu of an individual permit for each alteration to an existing electrical, gas,
mechanical, plumbing or interior nonstructural office system(s), the building official is authorized to issue an
annual permit for any occupancy to facilitate routine or emergency service, repair, refurbishing, minor renovations
of service systems or manufacturing equipment installations/relocations. The building official shall be notified of
major changes and shall retain the right to make inspections at the facility site as deemed necessary. An annual
facility permit shall be assessed with an annual fee and shall be valid for one year from date of issuance. A
separate permit shall be obtained for each facility and for each construction trade, as applicable. The permit
application shall contain a general description of the parameters of work intended to be performed during the
year.105.1.2 Annual permit records. The person to whom an annual permit is issued shall keep a detailed record
of alterations made under such annual permit. The building official shall have access to such records at all times or
such records shall be filed with the building official as designated.

1. In accordance with 110.6 of the Florida Building Code, “Work shall not be done beyond the point
indicated in each successive inspection without first obtaining the approval of the building
official.”
2. Inspection requests shall be submitted by e-mail to codeenforcement@duvalschools.org in the
format prescribed by the DCPS Code Enforcement Standard Operating Procedures.
3. “Any portions that do not comply shall be corrected and such portion shall not be covered or
concealed until authorized by the building official.”
