Introduction
The all-in-one Symphion System is designed to transform the way you remove the uterine tissue. It unites
breakthrough fluid management, pressure monitoring and bladeless technologies.
The Boston Scientific Genesys HTA™ System therapy is an outpatient procedure that uses heated saline to treat
the uterine lining, or endometrium. Once treated, most women no longer experience the symptoms or effects of
a heavy period. Many women stop having their period altogether.
Ordering Numbers
Boston Scientific Corporation uses a fully computerized order entry
system for the convenience of our customers. The system enables
Customer Service Representatives to provide you with instant order

The system uses UPN order numbers or GTIN numbers for each product.
The bold blue numbers are the old order numbers (see example). Please

STANDING ORDERS Boston Scientific accepts standing orders on all products. This convenience provides you with scheduled shipments using a single purchase order.

TERMS Terms are Net 30 Days. All shipments are F.O.B. Quincy, Massachusetts. Prices are subject to change without notice.
RETURNED GOODS Permission to return merchandise is required for all returns and may be withheld in the sole discretion of Boston Scientific.
For authorization, please contact the Customer Service Returns Department for a Return Goods Authorization (RGA) number at 1.844.664.6496.
The request for authorization must include:
– Reason for the return
– Order number, lot number, quantity
– Purchase order number, Boston Scientific invoice number, date of invoice
A copy of the packing slip or invoice must accompany all merchandise returned to Boston Scientific. The Returned Goods Authorization (RGA) number must be clearly marked on
the shipping container. Merchandise returned without an RGA number will not be accepted or considered returned for purposes of the warranty.

Merchandise processed in error by Boston Scientific will receive full credit if returned within 30 days of invoice date in saleable condition.
Other merchandise returned within 30 days of invoice date in saleable condition will receive full credit less a 10% Restocking Charge.
Merchandise returned past 31 days in saleable condition will receive full credit less a 30% Restocking Charge.
Material that is opened, damaged, expired or tampered with is not returnable for credit nor is product with less than six months of shelf life remaining.
Products manufactured as Specials or Custom products are not returnable for credit.
