The Faculty and staff want to welcome you as you begin your career as a nursing student. This Undergraduate Academic Policies Handbook has been developed to assist you toward the achievement of a Bachelor of Science degree in nursing. The School of Nursing Academic Policies are in addition to the policies specified in the Salem State University Catalog. To access the catalog online, go to the University website at www.salemstate.edu. The School of Nursing Academic Policies are updated annually. Your year of admission to the nursing major corresponds to the date of the Academic Policies that you must adhere to. Additionally, there may be policy changes that will impact all students regardless of the admission year. Students will be notified of these changes prior to their implementation. i.e. passing Grade for Medication Exam. It is the student’s responsibility to read all of the academic policies found in this handbook. DEFINITION OF TERMS Generic Student Undergraduate Advanced Placement Student LPN/BSN Student RN/BSN Student Flow Sheet Course Sequence Course Sequence Change And/Or Course Withdrawal Core Requirements Pre-Requisite Course Co-Requisite Course Support Courses Major Course An individual who enters the freshman year with no college credits and who follows the traditional undergraduate program of studies. ●An individual who enters the nursing major with existing college credit either as an internal transfer or from another accredited college or university. ●An individual who already has an undergraduate or graduate degree in another major ●An individual enrolled in the LPN/BSN Program as an Advanced Placement Student in the generic day program. ●An individual enrolled in the RN/BSN Program as an Advanced Placement Student in the School of Continuing and Professional Studies. An individual who has already achieved the status of an LPN who has matriculated into the LPN/BSN Program. An individual who has already achieved the status of an RN who has matriculated into the RN/BSN Program A table containing the catalog number, course name and proper sequence to be followed for successful completion of the program of studies. Flow sheets are found in Section Four of this Handbook. The correct order of course completion with the minimal acceptable grade. . For example, NUR370 is to be taken concurrently with NUR320 and the student must achieve a C+ in both courses. This information is identified on the flow sheet.and the course sequence should be adhered to as closely as possible. Students who for any reason, find it necessary to alter their Program of Studies course sequence or withdraw from a course must consult with their School of Nursing faculty advisor or the Department Chairperson or if in the RN/BSN and LPN/BSN Program, the Coordinator of Post-Licensure Programs. The consultation must be done before changing their course sequence or initiating a course withdrawal. Failure to do so may jeopardize their standing in the program and prevent progression to successful completion. Identified in the Salem State University Catalog as "a group of courses in various fundamental areas of knowledge which are required of all students". These core requirements include certain competency based skills which are also identified in the above mentioned catalog. These are courses which must be successfully completed according to the School of Nursing Academic Policies before entering certain support courses. Additionally, prerequisite nursing courses are also designated for the required sequence of nursing courses. Courses which are to be taken concurrently with a science course or nursing course. Courses to be taken by all nursing majors. They will be identified as either a pre-requisite or co-requisite course. Nursing courses which must be taken in proper sequence to complete the nursing major. Progression to the next level of courses depends on successful completion of each course according to School of Nursing policies. 1 DEFINITION OF TERMS Good Moral Character Eligibility to take the Professional Licensure Examination includes compliance with the “Good Moral Character” licensure requirement established in Massachusetts General Laws, Chapter 112, Section 74 (Board of Registration Policy 00-01). This includes evaluation by the Board of Registration in Nursing of all misdemeanor and felony convictions, and discipline by a licensure/certification body. Individuals convicted of certain crimes are permanently excluded from nurse licensure in Massachusetts. Applicants are advised to call the Board of Registration in Nursing at (617) 727-9961 for further clarification. Academic Integrity Salem State University assumes that all students come to the University with serious educational intent and expects them to be mature, responsible individuals who will exhibit high standards of honesty and personal conduct in their academic life. All members of the Salem State University academic community have a responsibility to ensure that scholastic honesty and academic integrity are safeguarded and maintained. Cheating and plagiarism are unfair, demoralizing, and demeaning to all of us. Cheating, plagiarism, and collusion in dishonest activities are serious acts that erode the University’s educational role and cheapen and diminish the learning experience not only for the perpetrators, but also for the entire community. It is expected that Salem State University students will understand and subscribe to the ideal of academic integrity and that they will be willing to bear individual responsibility for their work. Materials (written or otherwise) submitted to fulfill academic requirements must represent a student’s own efforts. Academic Dishonesty Policy The fundamental purpose of this policy is to emphasize that any act of academic dishonesty attempted by any Salem State University student is unacceptable and will not be tolerated. Charges of academic dishonesty are reviewed through a process that allows for student learning and impartial review. Discipline for academically dishonest behavior is exercised on two levels: without a formal hearing or with a formal hearing. A faculty member discovering an incidence of academic dishonesty must notify the School of Nursing Chairperson for day students and the Coordinator of the Post Licensure Program for RN/BSN students. Spring semester - by May 31 5 MANDATORY REQUIREMENTS FOR ALL STUDENTS PRIOR TO CLINICAL PRACTICE Prior to each academic year, the student must present the Compliance Officer with documented proof of the following: 1. Student Liability Insurance: The insurance fee will be billed directly to the student’s account Liability & every fall. If a student begins clinical in the spring they will be charged one-half of the amount Health by the Bursar’s Office Insurance 2. Health Insurance: Insurance may be purchase through Salem State or via private insurer. 1. Current certification in Basic Life Support for Health Care Providers. CPR Certification 2. If certification is due to expire prior to course completion, the student must present evidence of renewal to the Compliance Coordinator, prior to the expiration date. 3. Students may not continue in clinical practice without valid certification. Days missed will be considered absences and may jeopardize program progression. DAY STUDENTS (Generic, Advanced Placement, LPN/ BSN) must bring a copy of their Health Insurance, Student Liability Insurance and Basic Life Support certification cards to the Compliance Officer before the start of the Fall semester of each academic year. Students, who, for any reason, do not take nursing courses in the Fall semester, must present the above copies at the beginning of the Spring semester. RN/BSN STUDENTS: must bring a copy of their Health Insurance, Student Liability Insurance and Basic Life Support certification cards to the Coordinator of Post Licensure Programs before the start of the Fall semester of each academic year. Students, who, for any reason, do not take nursing courses in the Fall semester, must present the above copies at the beginning of the Spring semester. 1. Each student must have a completed copy of the Salem State University Health and Health and Immunization Form on file in the Health Services Office. The form is available online at Immunization http://www.salemstate.edu/chs/new.php. Requirements: a. Generic and Advanced Placement Students: Prior to entrance in NUR210B b. LPN/BSN and RN/BSN Students: Prior to entering a clinical course. 2. The completed form must provide proof of the student's immunization status as follows: a. a. T.D. (Tetanus/Diphtheria): Within 5 years or a Tdap booster will be required. b. b. MMR(Measles, Mumps, Rubella): one MMR and a second Measles vaccine after c. 1980 or a MMR titer. d. c. Varicella (Chicken Pox): positive IGG titer indicating immunity or 2 doses of vaccine. e. d. TB (Mantoux): ANNUAL REQUIREMENT or as specified by clinical agency. If positive f. TB test, a copy of a written X-ray report and an annual screening form. g. e. Hepatitis B Vaccine: 3 dose vaccination or Hepatitis B serology (HBsAB) value indicating h. immunity. i. f. Physical examination on file within the last four years. Some students may be required j. to provide this annually due to Clinical Agency Requirements. k. g. Clinical agencies may have additional vaccination requirements for practice such as the l. influenza vaccine on an annual basis. m. 3. Meningococcal Vaccine is required for all students living in the residence halls. Please read the information on meningococcal disease and vaccine at http://www.salemstate.edu/chs/new.php. Students may waive the vaccination, although we recommend that you obtain the vaccination. 4. Clinical agencies will not allow any student into their agencies without documented evidence of proper immunizations. a. Any student who does not have a complete, up to date record of immunizations will not be allowed to progress in the clinical rotation. b. Days missed will be documented absences and may jeopardize the student’s satisfactory completion of said rotation. In accordance with the Massachusetts Law and SSU SON policy, all nursing students will be CORI required to have a Criminal Offenders Record Investigation (CORI) conducted prior to the start of their first clinical experience. Additionally, clinical agencies may require more frequent CORI checks and/or a National Background check administered through HireRight. Students failing to meet the CORI requirement cannot complete the clinical requirements of the nursing program and therefore are ineligible to remain in the nursing major. 6 NURSING COURSE AND CLINICAL REQUIREMENTS Equipment Requirements for All Lab/Clinical Nursing Courses Clinical Dress Codes And Equipment for Generic & Advanced Placement Students 1. watch band with second hand 2. bandage scissors and clamp in accordance with agency policy 3. stethoscope 4. pen with black and red ink 5. calculator (optional) NOTE: only a specified four function calculator, available at the Salem State University Bookstore, will be allowed in the classroom and clinical area. No other electronic devices, (PDA, cell phones, etc.) will be allowed during quizzes and exams. Appropriate dress includes: 1. School uniform, Navy Blue Scrub Top, Navy Blue Scrub bottoms and/or white lab coat with the Salem State University emblem and/or as determined by agency request 2. Uniform should be cleaned and ironed 3. Clean and/or polished white leather sneakers or shoes and white hose. No backless clogs. 4. Name pin./ID clearly displayed 5. Daily hygiene with hair off collar 6. Appropriate length nails and no artificial nails. Only unchipped nail polish-clear or pale pink. 7. Minimal jewelry with no dangling earrings 8. No facial/tongue piercing or visible tattoos 9. Minimal cosmetics. No perfume or fragrances. 10. Stethoscope in clip or pocket, not around neck Note: Students must also adhere to agency dress codes. Transportation See course syllabi for additional specific equipment that must be purchased. Students are responsible for providing their own transportation to clinical placements. 7 FORMATIVE CLINICAL EVALUATION POLICY Unsatisfactory Student Performance Clinical Contract When a faculty member determines that a student is not meeting the clinical objectives, the following procedure will be implemented: Clinical Contracts are implemented for at risk students. 1. An at risk student is an individual who a. already has a critical element failure and/or b. has a significant number of general competencies that need improvement and/or c. has not met the performance requirements specified on their signed SBAR form 2. Faculty will: a. develop a clinical contract specifying the general competencies that need improvement b. review the contract with the student c. refer the student to the simulation laboratory for skills development 3. The contract will be dated and signed by both the student and the faculty member. A copy will be: a. provided to the student and course coordinator b . will be attached to the student’s clinical evaluation tool. 4. If the student remains on clinical contract after the summative final clinical evaluation the clinical instructor will notify the course coordinator. 5. The course coordinator will then notify the subsequent course coordinator. 6. The subsequent course coordinator will inform the assigned clinical instructor of an existing clinical contract from the previous course. 7. The assigned clinical instructor will prepare a preclinical contract and: a. meet with the student to review general competencies needing improvement that were identified from the previous clinical contract. b. the preclinical contract will be dated and signed by both the student and the faculty member. A copy will be: i. provided to the student and course coordinator ii. will be attached to the student’s clinical evaluation tool. 1. the faculty member will hold an individual conference with the student to identify: a. student performance issues/concerns b. expected level of student performance c. strategies to improve student performance d. target date for re-evaluation of student progress 2. the faculty member will: a. provide a written summary of the above using the SON SBAR format/form. b. identify if the summary constitutes a: i. verbal warning ii. written warning iii. critical element failure c. documentation of unsatisfactory student clinical performance need not be given in sequential order. The specific warning/critical element failure is determined by the individual situation. d. notify the course coordinator 3. the summary will be dated and signed (either electronically or by hard copy) by both the student and the faculty member. A copy will be: a. provided to the student and course coordinator b. will be attached to the student’s clinical evaluation tool. 4. if the student's performance re-evaluation indicates the student continues to not meet the clinical objectives, the faculty member will issue a clinical contract. 8 FAILURE OF A COURSE WITH BOTH A THEORETICAL AND A CLINICAL COMPONENT NOTE: Students must pass both theory and clinical components to pass the course Theoretical Component Failure: Clinical Component Failure: 1. A course theoretical component failure is determined by student achievement of the specified course evaluative methodology. 2. A theoretical component failure constitutes a course failure. 3. Before progression in the nursing major, the student must rectify the course failure. 4. No special waivers in academic progression will be granted for failure of the theoretical component of a nursing course. 1. A course clinical component failure is determined by student achievement of the specified course clinical evaluation tool evaluative methodology. 2. Faculty anecdotal records will be utilize to document student progress in meeting the objectives and indicator behaviors identified in the Course Clinical Evaluation Tool. 3. The tool: a. is given to each student at the beginning of each course and is used for the entire course. b. contains Critical Elements, key practices/indicator behaviors identified by School of Nursing faculty as essential for safe nursing practice. c. specifies the maximum number of critical element failures which would result in a clinical component failure. 4. Students not performing at the expected level will be counseled by faculty as identified in the Formative Clinical Evaluation Policy as specified on page 7. 5. If a student attains the maximum number of critical elements at anytime during the course, the student will not be allowed to complete the course as this constitutes a clinical component failure. 6. A clinical component failure constitutes a course failure. a. the student will be verbally notified before the next clinical day by his/her clinical faculty member. b. the student will also be notified in writing that the failure has occurred and copy will be placed in the student’s file as well as. forwarded to both the School of Nursing Chairperson and Associate Dean. c. the student who fails will be strongly encouraged to make an appointment with his/her faculty advisor to obtain the guidance pertinent to their future in the program/university. 7. No special waivers in academic progression will be granted for failure of the clinical component of a nursing course. Simulation Lab Critical Element Failure When the student is scheduled for a course required SimLab learning experience and is not able to attend, the student must notify the lab personnel of the absence prior to the scheduled appointment. It is the responsibility of the student to re-schedule the SimLab experience with the SimLab personnel within the required time frame for course completion. Failure to comply with this policy will result in a critical element failure on the student’s clinical evaluation tool. Failure to reschedule and attend the mandatory SIM lab session will result in a course failure. 9 SECTION TWO: GENERIC AND ADVANCED PLACEMENT STUDENTS Identifying Advisor & Scheduling an Appointment Preregistration Academic Advising Changing Course Sequence: Transfer Evaluation: Test Policy For Nursing Courses Academic Probation Policy: Student Leave-Of-Absence Policy: 1. Each student, enrolled as an undergraduate student, will have a School of Nursing faculty advisor. 2. A list of students and their advisors is posted on the second floor of the Harrington Building prior to the Fall and Spring registration period. (See university newspaper for dates). 3. Faculty have advising hours posted on their office doors and are available during these times unless unforeseen circumstances arise which prevent them from being there. 4. To schedule an appointment with a faculty advisor, outside of office hours, leave a note with your request and phone number in the appropriate faculty mailbox (located in HB 237) or leave a voice mail or e-mail message. 1. Students must make an appointment with their faculty advisor prior to registration for the following semester. 2. To facilitate the registration process, advisors place an appointment schedule on their office door approximately one week prior to the beginning of the Academic Advising Period. 3. Students are urged to make an appointment with their advisor as early as possible, regardless of their class designation. If you encounter difficulty in identifying a time for conference, leave your advisor a note, voicemail or e-mail which includes your telephone number. 4. Students should bring to their appointment a list of courses with they wish to register for with corresponding course & catalog number. 5. The faculty advisor discusses and approves the course selection, signs the registration form and provides the code number for the student to register in Navigator. Students will not be given a registration code without meeting with their advisor You are strongly encouraged to discuss any changes, such as withdrawal from a course, with your advisor, prior to action. No reminders will be sent to the student by the School of Nursing. 10 SECTION THREE: LPN/BSN PROGRAM POLICIES Granting Advanced Placement &/or Exemptions NUR320 Clinical Policy NUR321 Clinical Policy Student Leave of Absence 1. Assessing previous learning through life experience requires a review: a. completed by the Coordinator of Post Licensure Programs and/or the Academic Faculty Advisor at the initial interview or at the advising session prior to enrollment in the first clinical nursing course, NUR320. b. comprised of current LPN license, current resume, two professional recommendations, evidence of CEU’s and professional development courses and one year work experience as an LPN. 2. Students who are admitted are granted 6 Advanced Placement Credits and are exempt From NUR104, NUR105, NUR210B and NUR212B based on graduation from a Practical Nurse Program and LPN licensure. 3. Exemption from an aspect of a nursing course: a. is based on current academic policies b. students will be required to take the full theory component of each clinical course. c. determination will be made by the Coordinator of Post Licensure Programs and/or the Academic Faculty Advisor. d. subsequent recommendations, following an advising session with the Coordinator of Post Licensure Programs and/or Academic Faculty Advisor, will be made to the faculty of the course for which the student is petitioning an exemption. e. the final decision for an exemption will be made by the course faculty in consultation with Coordinator of Post Licensure Programs and/or the Academic Faculty Advisor in collaboration with the School of Nursing Chairperson. 4. For the 400 Level courses, following an advising session with the Coordinator of Post Licensure Programs and/or Academic Faculty Advisor, recommendations will be made to the faculty of the course which the LPN is petitioning an accommodation as follows: a. the student must provide documentation to validate having met course objectives warranting consideration for exemption. b. the documentation must be submitted at least one semester prior to the specific nursing course. c. the final decision for an exemption will be made by the course faculty in consultation with Coordinator of Post Licensure Programs and/or Academic Faculty Advisor and the School of Nursing Chairperson. 5. The Coordinator of Post Licensure Programs and/or Academic Faculty Advisor will maintain a record of advising sessions, recommendations and final decisions regarding specific exemption decisions and student academic progression/outcomes. For example: NUR320 faculty will document if there is an approved reduction in clinical days from two to one per week. Students are required to attend 2 days of orientation then will complete one clinical day per week for the remainder of the course. Based on individual clinical performance, the faculty member may require students to complete additional clinical days to meet the objectives of the course. Students will be required to attend 2 days of acute care clinical per week for the first 3-4 weeks. Based on faculty evaluation of student clinical performance, students who meet clinical course objectives during the first 3-4 weeks may be exempt from clinical for the remaining weeks of the course. A minimum of three, in person, Advisor Conferences are required of all matriculated RN students. These conferences are in addition to the initial interview and are to be scheduled as follows: 1. during the semester following acceptance to program. 2. during the semester preceding enrollment in NUR360A or NUR361A. 3. during the last semester, and before February 1, of graduation year. This is a final review of your program which is required to receive approval for graduation. 4. It is also highly recommended that students contact the post licensure coordinator for permission to take all nursing courses. Students who are admitted are granted 35 Advanced Placement Credits based on graduation from a nationally accredited School of Nursing and a current Massachusetts RN license. The student must submit a copy of current MA RN licensure throughout their program of study to the Coordinator of Post Licensure Programs. Students with knowledge that is acquired through alternative methods may apply for Life Experience Credit. Contact the Coordinator of Post Licensure Programs for further details. Nursing Research should be taken prior to nursing management and public health courses.
