This addendum is issued prior to the close of tenders to provide certain additional instructions
and/or revisions.

If the Bidder wishes to change the unit price schedule after his tender is submitted, he may send a
facsimile (fax) message to the following individual on a copy of the TENDER AMENDMENT FORM
included in this tender document:

To be acceptable, the change must be received no later than 1:30 p.m. on the closing date shown for the
tenders on this contract.
If a bidder sends a number of faxes each with a different change, only the last one received will be read out
at the opening. All of the others will be destroyed. If some of the changes on the previously submitted
faxes are to remain in effect, they must be repeatedly included in each subsequent fax that is submitted.
