OCTOBER 1, 2014 – SEPTEMBER 30, 2015 ANNUAL REPORT
LETTER FROM THE EXECUTIVE DIRECTOR
Welcome to our 2014 – 2015 annual report.
I’m pleased to have this opportunity to
share highlights of our last year at Family
Supportive Housing. It was a year of strategic
and programmatic growth. We expanded our
services and set ambitious new goals for our
work guiding families from crisis to
self-sufficiency.
As you will read, we served more than 200
families during this time. 73% of our client
families found permanent housing — a
remarkable statistic, given the South Bay’s
tight housing market. 48% of the adults
we served increased their income levels.
These numbers attest to the hard work that
we see families do every day to return to
independent lives in the community.
Our staff and Board work equally hard to
ensure that our programs keep pace with
families’ changing needs, and deliver real
impact that lasts. In early 2015, we started a
strategic planning process to evaluate our
performance and set priorities through 2018.
The resulting Plan (available online) defines
goals for a Superior Service Model, Effective
Advocacy, Committed Partnerships, and TopQuality Operations and Management.
1
2015 also saw the launch of an improved
and expanded Bridges AfterCare program.
AfterCare now allows families who
successfully complete our Shelter program
to “opt in” for extended case management
and access to supportive services, for up to
24 months after they move into permanent
housing. This innovative two-phase service
model aims to reduce the length of shelter
stays and to ensure that the families we serve
do not return to homelessness.
Our continuing efforts to help homeless
families were recognized, when Family
Supportive Housing was selected as a 2015
Bank of America “Neighborhood Builder.”
You can read more about this prestigious
award on page 3.
As you review highlights of our recent
work and impact, I invite you to get
involved. Together, we can help families put
homelessness behind them, forever.
Beth Leary
Executive Director
ABOUT FAMILY SUPPORTIVE HOUSING
Family Supportive Housing helps
homeless families with children
remain intact while addressing their
needs for food, shelter, employment
and education. We offer families an
opportunity to make life-altering
changes and a system to aid in finding
permanent housing.
Bridges AfterCare offers families,
who successfully complete the Shelter
program, extended Case Management
and supportive services for up to 24
months after they move into permanent
housing. AfterCare helps families to
establish a strong foundation for longterm success and stability.
San José Family Shelter is the only
emergency shelter in Santa Clara County
that does not separate the family.
It can accommodate 35 families, each
in a private room with bath, for up
to 90 days, as they work to become
self-sufficient. Onsite Case Managers
provide counseling, referrals for housing,
employment services, budgeting
services, health care services, workshops,
parenting classes, and ESL education.
Supportive services for children include
an onsite, licensed Child Development
Center for infants and pre-school
children, and a Homework Enrichment
Program to keep K-12 students on track
for success.
Glenn Art Arms Transitional
Housing Program offers scattered-site
apartments for families who pay 30% of
their income toward rent. Families also
receive supportive services that focus
on increasing income and education to
support independent living.

PRESTIGIOUS NATIONWIDE GRANT CREATES LASTING
IMPACT IN LOCAL COMMUNITY
Family Supportive Housing (FSH) is
honored to be a recipient of Bank
of America 2015 Neighborhood
Builders grant. The grant allows FSH to
strategically grow within Santa Clara
County, increase its presence, and
empower it to meet the needs of families
experiencing homelessness.
In addition to funding, Bank of America
is also providing development training
for the Executive Director and the
Director of Operations, Christi Moyer
Kelly. Christi will interact with leaders
from other organizations and learn
new techniques which will be used to
improve FSH’s services.
Benefits of the grant include improving
FSH internal controls, procedures, and
quality of services. This will increase
the organizations ability to adapt,
scale and innovate FSH programs, as
well as improve the measurement and
evaluation program outcomes.
With the extreme shortage of affordable
housing and rise in children living
below the Federal Poverty Level, FSH
is receiving an increasing demand for
services, while making the services
more difficult to achieve. These trainings
will help FSH understand its emerging
strategic needs, allow FSH to develop
and nurture partnerships with other
Neighborhood Builders, introduce
effective tools and techniques to spur
community support, provide skills for
reflecting approaches for serving the
community, and expose FSH to best
practices for emulating and improving
outcomes.
FSH will also be able to recruit and train
highly qualified staff, which will allow
Executive Director, Beth Leary, the ability
to be more active in the community,
connect with existing and prospective
funders, and leverage the Neighborhood
Builders funding to attract additional
support. Expanding FSH reach in the
community will increase financial
sustainability.
3
FSH looks forward to leveraging the
grant to increase the impact it has on the
community.
