lnstructions for Sending and Recording Gifts
CRC Form (Kennel Proiect) and Donor Card
lnformation
To ensure proper and timely acknowledgement of gifts, Clubs are asked to follow a uniform process in
recording their campaign gifts. By following the guidelines outlined in this document, data entry errors
will be minimized and gifts to the campaign will not be confused with other contributions to Leader
Dogs for the Blind.
Please Note
Many gifts will be received as pledges or long-term commitments payable over three to five years.
Careful recording of these, together with the donors' requested method of payment, is essential.
Leader Dog has implemented a centralized system to receive pledge payments and also mail statements
to pled3e donors. This process can only be effective when Ciubs provide accurate information that can
be properly recorded. Clubs are asked to use the Club Record of Contribution (CRC Form-Kennel
Project). Use of these will assure proper credit.

Please complete Club Name, number of members, district, prepared by information, date prepared,

Donor Status
o lf a Club Gift or proceeds raised from a fund raiser will be used please indicate by circling- Club

Donor lnformation
List the name in the exact manner in which the donor wishes
to be addressed in their acknowledgment
letter. Enter the complete address, including City, State and Zip Code.
There is also a space available for the donor to indicate how they would like their name to appear in
campaign recognition materials, if it is different than how they listed in the Donor lnformation.

Enter the total gift or pledge amount in the boxes provided under to Total Gift/Pledge Amount.
Cash Pavment Enclosed
Enter the amount of the check or money order payment (one-time gift, initial pledge payment, or
payn,ent on an existing pledge) that is being sent at the time of the gift. The check or money order must
