NOTICE OF PRIVACY PRACTICES
ONLINE PRIVACY POLICY
The Clinic respects your privacy rights as an online visitor to the Site and recognizes the
importance of protecting any information collected about you. This Privacy Policy guides
how the Clinic collects, stores and uses online information provided by you to us on this
Site only. Please be aware that other Clinic web sites or pages that you may access
have other online privacy policies. This Privacy Policy is used to supplement the HIPAA
mandated NPP set forth above. In the case of any conflict between this Privacy Policy
and the NPP, the terms set forth in the NPP shall prevail.
What information is collected automatically?
The Clinic may collect IP addresses (the Internet address of a computer) to track a
user’s session while the user remains anonymous. We may analyze this data for certain
trends in statistics, such as how long users spend on the Site. We do not link your IP
addresses to anything personally identifiable to you, except when you enter the Risk
Assessment. The uses and disclosures and your rights with respect to all personal
information collected from you from the Risk Assessment is covered by the NPP.
Is the information that I provide safe?
The Clinic maintains appropriate administrative, technical and physical safeguards to
ensure the confidentiality, availability and integrity of the information that you provide us
as required by the HIPAA Security Rule and applicable state law.
Do you use cookies?
Cookies are small pieces of data that are sent from web servers to your browser and
stored on your computer’s hard drive. Cookies cannot access other data on your hard
drive and will not damage your computer in any way. The Site limits the use of cookies
for only storing session state information. Session state information is gathered in the
Site so that as you move from page to page within the Site, your interaction with the Site
can be appropriately modified without the need of asking for personal information
repeatedly. Session state information is only kept during a browser session and is
automatically deleted by your browser at the end of every session.
What is your policy for collecting information from children?
The Site is focused on providing a healthy program that benefits individuals 16 years
and older. This is an online environment in which we would neither want nor expect any
type of information from children. The Clinic will not knowingly collect information from
anyone under the age of 16. If at any time the Clinic becomes aware that it has received
information from someone who is under 16 that information will be immediately
removed.
