Account Application Instructions
General Information
IMPORTANT: If you are unable to provide two (2) primary forms of identification,
you may provide one (1) primary form, as listed above, and two (2) secondary
forms of identification.
All sections of the application must be completed. Your account cannot be opened
unless all requested information is provided.

• If areas of information are the same as that of the Primary Account
Holder, the Co-Account/New Account Holder may write “SAME” in the
respective section of the application. Please do not leave it blank.
• You must provide a residential street address in your account application. “P.O.
Box,” “In Care of,” or “Business” addresses cannot be accepted. You may
provide a corporate address or a personal P.O. Box as your mailing address only.
• Please make sure to complete “Address since” field for your permanent
residential address and, where applicable, your previous permanent
residential address.
• Your home telephone number must be the one at your residential street
address. If there is no phone number available, please indicate “N/A”.
Identification
Each account holder must provide information from two (2) primary/
government issued forms of identification. Please make sure to complete all
fields of information requested.
Primary* Forms of Identification include:
• Valid passport with picture and signature
• Valid national ID card with picture, date of birth, signature and with
an acceptable issued and expiration date
Banking Card and Checkbooks
Please indicate whether you would like to receive a Citibank Banking Card (ATM Card.)
You can order checks once your account is funded.
Signature Cards
Each Account Holder must sign both signature cards. The signature(s) of every
account holder must appear on both signature cards.
Additional Documentation
• If you are employed by your company for less than one (1) year, you will be asked
for additional documentation at time of account application.
• If Co-Account Holder is not employed, additional documentation will be requested
at time of account application.

Secondary* Forms of Identification include:
• Alien Registration / Identification Card
• Valid credit card with a photograph issued by a recognized financial
institution
• Certified Birth Certificate (Official certified copy only, photocopies
not acceptable)
*Other primary or secondary identification may be acceptable (e.g. driver's license) depending on your country of citizenship or residency. Contact us to find out other types of acceptable identification.
