form. Failure to do so may subject the proposal to be considered as non-responsive.

1. If 15% minority participation is not obtainable due to union labor restrictions, then an
explanation needs to be provided in the proposal and another commitment percentage offered
2. All firms need to be prequalified with PSU and Barton Malow. The PSU list is posted on
their website. Barton Malow will follow-up with those firms that need to complete our prequal
process (Nick Umosella is the contact for Barton Malow).
3. Dates for All three stages of submission will be revised to January. Please see Addendum 1
