MEMBERSHIP CONTRACT
In consideration for a one (1) year membership to the GREEN TREE TENNIS CLUB, I/We hereby agree to pay
the following annual membership fees, (as indicated by the box checked below), which is payable in one lump sum or in 12
equal monthly payments:

annually or $78.00/month for 12 months on or before the first day of each month as hereunder provided. Prices do not
include sales tax.

for 12 months on or before the first day of each month as hereunder provided. Prices do not include sales tax.

READ CAREFULLY
I UNDERSTAND THAT IF I ELECT TO PAY THE MEMBERSHIP DUES ON THE MONTHLY INSTALLMENT
BASIS AND DEFAULT IN THE PAYMENT OF ANY INSTALLMENT THEREBY DUE, THEN
AUTOMATICALLY AND WITHOUT OR DEMAND, THE WHOLE AMOUNT OF THE ANNUAL MEMERSHIP
DUES AND ANY OTHER CHARGES THEN UNPAID AT THE TIME OF DEFAULT SHALL BECOME
IMMEDIATELY DUE AND PAYABLE. CREDIT INQUIRIES ARE AUTHORIZED WHEN THE ACCOUNT IS
PAST DUE. COLLECTION EXPENSES WILL BE PAID BY THE APPLICANT. I AUTHORIZE A
REPRESENTATIVE OF GREEN TREE TENNIS CLUB TO CHARGE THE BALANCE OF YEARLY UNPAID
DUES AND OTHER UNPAID CHARGES TO MY CREDIT CARD (LISTED ABOVE) or TO DEBIT THE UNPAID
BALANCE FROM MY CHECKING ACCOUNT. NOTE: Yearly membership will auto-renew unless
written notice (e-mail or other written form) is received 15 days after renewal date.
Private food and drink are allowed in the picnic area only.
In consideration for the use of Green Tree facilities, the above listed applicant does Release,
Acquit, Discharge and agree to Indemnify and hold Green Tree Tennis Club harmless on account
of or in any way related to applicant’s use of club facilities, or use, consumption or serving of any
alcoholic beverage at Green Tree and/or in conjunction with applicant’s activities at Green Tree.
