Instructions:
1. Please use a separate form for each individual PO number. Click the "Highlight Fields" or "Show fields" button at top right to make the
form fields visible. You can order up to five parts (with the same PO number) per form.
2. If a finished (non-hardware) part is requested for replacement, please include photos of the defective part (unless part is missing). All
requests without pictures will be considered incomplete and will cause delays in processing the order.
3. To submit a completed form, first save the form to your computer (use "Save As" in the File menu.) Then do one of the following:
• Email: Click the "Submit via Email" button. This method is intended for those using a desktop email client such as Microsoft Outlook
(PC) or Mail (Mac). If you use a Web-based email client (Gmail, Hotmail, etc.), you can attach the saved form to an email manually and
email it to PartsDept@aspenhome.net. Be sure to also attach any relevant photos (see #2 above).
• Fax: Click the "Print Form" button. Please fax the completed form to (888) 484-5874, along with any relevant photos (see #2 above).
Please ensure that both pages of the form are faxed.
