School Vision Clinic
HWDSB is offering students the opportunity to receive full eye exams during school hours.
The exams will be free of charge and will be provided by Creative Vision Cares which offers a mobile service that will visit your child’s school during our Vision Week from Monday
February 22 to Thursday, February 25. The service is run by Dr. Jonathan Hepburn and associates. The eye exams will consist of the following tests:
* Measuring your child’s current vision
* Testing colour vision and depth perception
* Assessing the ability of the eye muscles to work together
* Measuring pupil function and size
* Testing for glasses
* Using a microscope to assess the health of the eye
Risks and Benefits
* The eye tests are not painful and do not carry any risks. If further assessment is required, it will be indicated on the information sheet that students will bring home following
the eye tests
* An eye exam for your child is very important, as the eye doctor will not only determine
your child's prescription for eyeglasses, but will also check your child’s eyes for common
eye diseases, assess how your child’s eyes work together as a team and evaluate your
child’s eyes as an indicator of his/her overall health
* If glasses are needed, Creative Vision Cares and HWDSB will make glasses available to
your child for a cost of $10.00 (if you are not able to provide $10, alternate arrangements
can be made)
* You may also choose to purchase the glasses on your own and will be given a prescription
to do so
Privacy
* Your child’s eye exam results will be stored and maintained by Creative Vision Cares
* The information will be kept private and confidential as defined in the Personal Health
Information Act
* To support your family, HWDSB would also like to receive the results of your child’s eye
exam
* You also have the right to request your child’s information kept by Creative Vision Cares,
at any time, by contacting Dr. Jonathan Hepburn at creativevisioncares@gmail.com or by
mail at Creative Vision Optical 282 Argyle Street South Caledonia, ON N3W 1K8.
IN ORDER FOR YOUR CHILD TO RECEIVE THIS SERVICE, YOU MUST COMPLETE AND
RETURN THE CONSENT FORM BY JANUARY 20, 2016.
The Kinders are selling cookies at first Nutrition Break from Monday, February 8thThursday, February 11th. Cookies are $2.00 each and there are some delicious flavours (Vanilla Heart Cookies, Chocolate Chip Delite, and Chocolate Caramel).

Chedoke’s Jump Rope for Heart fundraising total is…….$1,000!! WOW! Chedoke JUST met their
goal! WAY TO GO!
Please find attached your Record Sheets. Don’t hesitate if you have any questions around it, please
note that each tab on the Excel document is a different class. Please be advised I DID NOT order prizes
as requested. I DID however order skipping ropes for the students who are marked on the sheet as
fundraising online. These sheets are simply for you to have your fundraising data info.
Chedoke earned: 1,068 HeartSmart Points! The Order Form is the last tab on the Prize Sheets document titled ‘HSP Order Form.’ The form can be emailed back or faxed to 905-574-4380. Let me know
if you have any questions and thank you SO MUCH again for such a successful event this year! It
was a pleasure working with you both and the whole Chedoke school community. We will be delivering
the student’s tax receipts shortly once they are completed. You will also be receiving “Thank You”
letters in the mail.

Eco Team News
So February kicks off today with Lights Out Lunches. Periodically members of the Eco Team
will be go around to check if lights and tech are off and handing out Energy Hogs or Light
Bulbs.
We are also having a poster contest for each division. The idea is to design a poster explaining
or showing how people can save energy or why it is important. Posters are due to the office by
February 22nd and will be judged by the Eco Team and Admin.
Cash Online Survey
Hamilton-Wentworth District School Board is pleased to offer School Cash Online, an
online service that allows you, as parents/guardians, to pay for your child's school fees
and purchase items related to their school experience in a secure and convenient way.
Our school is enrolled with School Cash Online. In order to help us improve the service
for families, HWDSB would appreciate feedback from you by completing a short survey,
which can be found on our school website.
The survey is voluntary and should take about 5 minutes to complete. All survey responses are anonymous and will be reported on in a summarized format.
The survey will remain open until the end of the day on February 12, 2016.

School Cash Online – What is MyWallet?
MyWallet is used when you want to load funds from your bank account in advance of paying for items online. The process of adding funds to MyWallet and paying for items are
two separate processes. It’s like buying a gift card and then using it later to pay for
items. There may be a slight time delay between when funds are loaded in your MyWallet
and when they are available for making purchases.
Funds may be retained in MyWallet over the summer. As students progress to other
HWDSB schools, all MyWallet funds follow the student. If necessary, refunds for unused
MyWallet balances may be requested via email to mywallet@kevgroup.com. Details on the
information requirements to obtain a refund may be found on the “Refund Policy” link on
the School Cash Online website.
