Instructions when unable to attend a Photo Point in Victoria
Authorised Photo Points are located only in Victoria. If you are not able to attend an Authorised Photo Point please
follow these instructions.
1. Obtain current colour passport size photo (45x35mm) with frontal view of head & shoulders, without head
covering or tinted glasses. Write your licence number (from your Payment Notice) on the back.
2. Take this form, your Payment Notice & photos to an authorised witness from one of the following professions

A person who holds an office in the Public
Service that are prescribed classifications for
the purpose of Section 107A of the Evidence
Act 1958. (a) Non-Executive employees
(VPS-2, VPS-3, VPS-4, VPS-5). (b) Executive
(Level 1, Level 2, Level 3).

Commissioned Officers currently serving in the
defence forces
Members of the Legislative Assembly of the
defence forces
Barristers
Registrars of Court

3. The witness must sign the back of the photo and write: “I certify that this is a true photograph of …………..”
4. Sign the Payment Notice in the presence of the witness.
5. The witness must then sign the following declaration:
I am satisfied that the person whose photograph I have witnessed is the same person named on the attached
Payment Notice by virtue of :
(eg. Drivers Licence, Passport etc.)
