Withdrawal from ALL Classes UCO ID Number: * Name (Last, First, MI): Date: Email: Cell: Address: City: Semester (Check one): Fall Spring Summer State: Year: ________ Zip: Intended Date to Return: PLEASE READ BEFORE COMPLETELY WITHDRAWING FROM SCHOOL The completion on a withdrawal from all classes after the semester begins does not necessarily indicate a clearance of financial or other obligations. All obligations of the University must be met regardless of the student’s withdrawal status. Refunds, if any, at the time of completion of withdrawal are according to the Regents’ refund policy as detailed in the class schedule. If your printout indicates a refund is due, you should receive your check by mail within (2) weeks. Withdrawing from school may change your student status. You should consider how your decision will affect your future status if you are receiving one of the following: financial aid, social security, veterans’ benefits, or a scholarship. Your ability to purchase insurance as a student, to maintain athletic eligibility, or to maintain a student visa (if an international student), may also be affected as a result of your class or school withdrawal. There is no record of the enrollment if withdrawing before the official beginning date of the semester. If withdrawing on or after the first day of the semester, but before the third week of a sixteen week semester, the notation “Withdrew” and the date will be made on the official transcript. Withdrawals beginning with the third week of a sixteen week semester will result in the grade of “W” appearing for each course dropped. If you have any doubt as to dropping a class or wonder if your withdrawal from school is in your best interest, please see your advisor (405-9742727) or make an appointment with a counselor (405-974-2215) in the Center for Counseling and Well-Being.
