Candidate Signature Page
Membership in an honor society such as National Honor Society is a privilege. The candidate forms, along with
Faculty and Staff recommendations, will be reviewed by Faculty Council. To be invited for membership means
that a candidate meets all four pillars of Scholarship, Character, Leadership, and Service.
I understand that completing and submitting the candidate forms does not guarantee selection to the National
Honor Society. I attest that the information presented is true, complete and accurate. If selected, I agree to abide
by the standards and guidelines of the chapter and to fulfill all of my membership obligations to the best of my
ability.
