IMPORTANT INFORMATION FOR APPLICANTS
1: CAR 5.09(1)(c) states that CASA must ensure an applicant is a fit and proper person before issuing a permission.
Note that an ASIC is current for a 2 year period and an aviation security check, required for an AVID, is valid for 5
years. The AFP security check is only valid for a period of 3 months. Please provide details in Section A6 of any
regulatory action, traffic and criminal charges, convictions or imprisonment. Action includes, but is not limited to,
suspension and cancellation of any aviation authorisation and includes Australian and overseas actions, charges,
convictions or imprisonment. This is still required, even if you hold a valid ASIC or AVID.
2: CAR 5.75(1)(b) states that before a student pilot attempts the GFPT flight test, they must hold an aircraft
endorsement on the aircraft used for the flight test, with the exception of an airship. Therefore, the Form 214 sticky
label for the aircraft endorsement and the applicable endorsement form (Form 187, 188, 203 or 204) must be
submitted to CASA either prior to, or with, this application.
3: This form can be completed electronically and saved locally to your computer. Once the ARN has been entered
into the first page, it will automatically update in the applicant ARN fields on subsequent pages. CASA recommends
that applicants complete as much of the form electronically as possible.
4: Please ensure your application and the checklist are completed correctly and that all required supporting
documentation is provided. Incomplete applications will not be accepted and may be returned to you for amendment.
