All sections of this form must be complete before it is submitted to the CLAS Dean's Office. Official CU Denver email can be used in place of a written signature if submitted electronically. The
date the form is received is the effective date of the action requested. Students and faculty will receive a confirmation email within 24 business hours of submitting this form. If you have not
received a confirmation email after 24 hours, please notify the Course Coordinators by sending an email to clas.courses@ucdenver.edu. Students must log into their UCD Access Portal once
they receive the email and complete their registration for this course, following the instructions they receive in the confirmation email. Please consult the academic calendars for specific
semester deadlines.

Faculty will be included in all email communication with student regarding registration completion.
Students should fill out this form and present it to the instructor of the course in which they wish to enroll, either in person or via official University email. If in person, the student should then
deliver this form with the instructor's signature to the CLAS Dean's Office in NC 5014 before published deadlines. If via email, the instructor of the course should forward the form and the email
chain from the student's official CU Denver email to clas.courses@ucdenver.edu before published deadlines, using their official CU Denver email address and cc'ing the student. Forms
submitted directly from the student's email address will not be processed. Within 24 business hours of submission, an email will be sent to the student and instructor, with instructions for
completing registration. If the instructor cannot use their CU Denver email address, they may forward the form to their department chair, who will then authorize the request by forwarding the
email chain and form to the Course Coordinators.
By signing this form, the instructor acknowledges that they are also requesting for the enrollment cap for their course to be increased. If the increase will over-fill a classroom, the request to add
the student will be denied, unless a larger classroom can be found. A copy of this form and all pertinent email communication will be saved in the Student Advisory Network (SAN) for
