involving the use of vertebrate animal subjects.
Provide a signed copy of the Assumption of Risk and General Release Form (provided under separate cover).
To minimize the risk and transmission of infectious diseases from infectious patients to others and to prevent
infection of research and laboratory staff exposed while conducting research activities, awardees are required to
provide a copy of a Risk Reduction Plan developed with his/her faculty mentor prior to international travel.
Awardees should acknowledge [insert name of granting body] in all presentations, publications and reports
derived from the funding. Please use the [insert name of granting body] logo in publications, presentations and
reports. Copies of publications for distribution must be made available to [insert name of granting body].
Awardees may be asked to present work at a [insert name of granting body] sponsored event or workshop for
awardees to share lessons learned, pool practical knowledge, and present progress on work.
A brief narrative report [no longer than [insert page max]], monograph, or other significant publication upon
completion of your work should be provided by [insert due date]. This report should include a list of any
publications or presentations made during the course of the award. Where possible, please include copies of the
aforementioned publications and presentations. Submission of this yearly narrative/progress and financial report is
due 30 days after your award ends. A template may be provided at a later date. Please note this summary may be
used to develop a press release, feature story or be used for development purposes. Images/photos are welcome to
accompany any stories developed.
Awardees may be invited to present a poster or research talk at a [insert name of granting body] sponsored
symposium.
In the event other funds are received to support your proposal, your level of effort changes, or your proposal aims
are significantly altered for any reason, you are obligated to notify [insert name of granting body].
Awardee shall be solely responsible for the payment of any and all taxes or fees, whether foreign or
domestic, relating to awardee's activities under this Agreement or receipt by awardee of compensation
therefore, including, without limitation, all federal, state, and local income taxes, all benefits and welfare
taxes or contributions required under the laws of any jurisdiction, and any professional or licensing fees
or other taxes or fees arising from awardee's activities hereunder.
