FUEL SURCHARGE PER MILE TABLE
Recently the price of fuel has escalated and it is necessary to institute a fuel surcharge to apply in
addition to our line-haul pricing.
In order to assist in offsetting the increase in fuel we agree that for each 5 cents per gallon increase in
the D.O.E. reported national average fuel price a 1 cent per mile will be added to the line-haul charges.
The surcharge will be implemented automatically as follows:

Should the price exceed $5.699 one additional cent shall be added for each 5 cents per gallon increase
in the D.O.E. reported fuel price. The surcharge will be adjusted each Wednesday based on the D.O.E.
price reported on Monday (2 days earlier). For example the price reported on Monday September 12,
2007 would determine the cents surcharge effective September 14, 2007. The surcharge shall fluctuate
with the reported D.O.E. average price.
The surcharge shall apply to line-haul freight charges incurred by you with the following companies of
the Jones Motor Group: Jones Motor Co., Inc. and Hot Shot Express, Inc.
