Our aims CHYP provides supported accommodation for single young people who are aged between 16 and 25 years old. We aim to offer stable housing with the support services to enable our residents to develop independent living skills and realise their full potential. We can help with things like ü ü ü ü ü ü ü ü ü ü ü ü ü ü ü ü Who we help We help young people who are homeless or in ‘housing need’, this might include people who are: • • • • • • Unable to remain in their family home Leaving foster care Unable to access private rented accommodation Homeless following a relationship breakdown Experiencing domestic abuse Wanting housing support or help with independent living skills People usually stay at CHYP for between 6 months and 2 years and everyone who comes to live at CHYP must be looking to move on to independent accommodation at some point in the future. General advice and support Finding training and job opportunities Preparing for independent living Coping with shopping and cooking Dealing with your finances Claiming benefits Understanding your rights and responsibilities Gaining an awareness of home security and personal safety Developing social skills and learning to live with others Helping you plan for your future Looking after your home Dealing with laundry Getting access to other services Helping you to find your own solutions Gaining confidence and motivation Advice on health living and wellbeing We cannot help with û Personal care – things like washing, dressing or eating û Going shopping for you – but we can go with you till you get the hang of doing a weekly shop on your own û Cooking meals for you – although we can help you learn to cook û Doing your laundry – but we can help by reminding you and showing you how to use the machine û Looking after your money – although we will help you to plan your spending or deal with any debts Planning your support We offer everyone an individual package of support. You just need to be willing to work with us. In order to work appropriately and effectively with the people we support, we need to: • Assess needs and risks • Identify goals and plan support • Record actions and achievements • Review progress To do these things we use a system called My Way Forward which was developed by Stonham Housing Association. Your file will include a Core Assessment, Support Assessment and Safety Plan that were drawn up during your assessment interview. After you move in you and your Keyworker will agree a Support Plan. You will then identify steps towards reaching your goals and look at what help you may need to do this and when this should happen. These things are recorded on a Goals Sheet. You will also have a Communication Record and Record of Contact that all staff fill in when they have seen you or helped you with something or discussed something with you. We need to do this so that we can see what type of help people need and how our staff can best meet these needs. You and your Keyworker will Review your Support Plan, Safety Plan and Goals at least once a year. Your review is a time to look at what has worked well and what has not worked so well; what needs to change and what should stay the same. Points to remember • You are the expert in making changes in your life - My Way Forward is a way of recording your views and choices and making sure that you get the support you need to make the best decisions for yourself. • You can see your file at any time - you have the right to see anything and everything that we record about you, whether this is handwritten or a computer record. • You are in control – if there have been any changes in your support needs or circumstances you can ask for a review or to change your support plan at any time. • If you have other people helping you like a social worker, family member or CPN we will work with them too if that is what you want.
