EN-18-201601 INSTRUCTIONS TO THE STUDENT 1. This form must be accomplished in quadruplicate (4 copies) and submitted to the Enrollment Services Hub when all (except University Registrar) necessary signatures have been completed. Application forms with incomplete information and signatures will not be accepted for processing. 2. Upon submission, the Enrollment Services Hub will make an assessment of the relevant fees for the application. The assessment will be printed in the Enrollment Assessment Form. 3. Pay the assessed amount at the Accounting Office. The Accounting Office will validate this application. 4. Submit three (3) Enrollment Services Hub. The fourth copy is the student’s copy. The Office of the University Registrar will transmit the copy for the academic department within two (2) working days from receipt hereof. 5. Coordinate the schedule and venue of the defense with the secretary of the academic department. 6. In case of final defense, submit the following to the secretary of the academic department for transmittal to the Enrollment Services Hub within the deadline: 6.1 Signed Approval Sheet 6.2 Five (5) Compact Disks, subject to the guidelines indicated in number 7 below. 6.3 Application to Graduate (Form No. S-01) 6.4 Photocopy of Official Receipt (corresponding to payment of graduation fees) 7. The following guidelines should be observed in the submission of CDs: 7.1 The version of the graduate thesis/dissertation contained in the CDs is consistent with that approved by the final defense panel; 7.2 The CDs are complete in contents, including annexes, tables and the like; 7.3 The contents are in non-editable PDF (Portable Document File) format; 7.4 The contents are readable in a standard computer; 7.5 The CDs used are of reputable brand; 7.6 Each CD must be signed by the faculty adviser using a permanent marker; 7.7 Each CD should have a transparent case and appropriately labelled in the manner displayed below: Title of Thesis/Dissertation Program (e.g., PhD in Development Studies) Name of School Name of Student (Last name, First name, Middle name) Month and year of completion (e.g., April 2006) Name of Faculty Adviser TERMS AND CONDITIONS 1. To be able to enroll for thesis/dissertation proposal/final defense, the student must be enrolled in thesis/dissertation writing course during the term. 2. The thesis/dissertation proposal defense may be enrolled up to the end of Week 9 of the term. The thesis/dissertation final defense may be enrolled up to the end of Week 7 of the term. 3. A defense resulting to revision requirements in the thesis/dissertation is classified as “Incomplete.” To qualify for completion, revisions must be approved and reported by the Chair of the Defense Panel to the Office of the University Registrar within three (3) terms from term of enrollment in defense. After this period, the “Incomplete” is automatically converted to “Failed,” in which case the student has to restart the thesis/dissertation cycle.
