Change of Ownership
I do hereby release ownership of my horse to the All Breed Rez-Q.
I understand that upon giving my horse to the above named rescue, that rescue
becomes the sole owner of the horse. I will make no attempt to reclaim my horse. I fully
grant this facility the right to give my horse any medical treatment deemed necessary,
and to possibly place the horse in a good adoptive home. Should a veterinarian
determine my horse is too ill to be treated, if necessary, I give the above named facility
the right to have said horse humanely euthanized. I release the above named facility
from all liability concerning said animal, and the above named facility will make every
attempt to provide adequate care and find a permanent home or keep said horse in
retirement for the life of said horse.
I HEREBY STATE THAT I AM GIVING UP MY HORSE OF MY OWN FREE WILL.
