MINIMUM ADMISSION STANDARDS FOR THE ASSOCIATE DEGREE NURSING PROGRAM INCLUDE: 1. Unconditional admission to the college. Refer to the current college catalog for detailed requirements. 2. Receipt of completed application for the Associate Degree Nursing Program. 3. Minimum of 2.5 GPA based on the following: a. most recent 24 credit hours of undergraduate work (if applicable). b. most recent 24 credit hours of graduate level work (if applicable). c. if less than 24 credit hours at graduate level –the most recent 24 undergraduate credit hours will be used-graduate credit hours will be ignored. d. a minimum of 2.50 high school GPA for students without prior college work (GED acceptable in lieu of high school transcript). 4. Eligibility for English 101 and Math100 or higher as determined by college policy. 5. . Good standing with the college. Check with admissions to make sure there are no holds on your account. 6. Meeting the essential functions or technical standards required for nursing. 7. Completion of the Test of Essential Academic Skills (TEAS V) must have been taken prior to application deadline. 8. NURSING PROGRAM CHECKLIST It is the student’s responsibility to submit a complete nursing admissions packet. Incomplete packets will be not accepted. Faxed or emailed packets will not be accepted. Students must initial beside each statement below to ensure that they meet the minimum admissions criteria. Meeting the minimum admissions criteria does not guarantee acceptance into the program. 1._______ Completed application to the nursing program. 2._______ It is the applicant’s responsibility to have ALL transcripts mailed directly from the sending institution, including nonaccredited schools. In order for your application to be complete, all documents, including official transcripts from EVERY institution you attended must be submitted to Enrollment Services. If the applicant is using any high school grades, high school transcripts must be submitted with (attached to) the nursing application. If the applicant has any graduate level courses, the official transcript from the awarding institution must be submitted with the nursing application. 3._______ Each applicant is required to take the TEAS-V (Test of Essential Academic Skills) The maximum score on the TEAS test is worth150 points and the test scores are good for three years. To register and obtain detailed information please visit www.atitesting.com. Students who have taken the TEAS -V test at another institution are responsible for ensuring test scores are received and on file by deadline. Unofficial Test scores will not be accepted. For the scores to be official they must be sent directly to the testing office. Please contact the testing office (205-856-7896) to check that scores have been received and loaded before the deadline. 4._______ GPA must be a 2.5 or higher from most recent 24 credit hours. See conditions under Minimum admissions standards above. 5._______ Full Time Program only: must be eligible to enroll in ENG 101 or have taken ENG101 and must be eligible to enroll in MTH 100 or higher or have taken MTH 100 or higher. Refer to the current Jefferson State Community College catalog for course prerequisites or placement test score requirement. Note: MTH 116 is no longer accepted for the nursing program and is actually a lower math class than MTH 100. 6._______ Part Time Program only: must have completed BIO 201, ENG 101, and MTH 100 or higher before applying to the program. Admission to the Associate Degree Nursing Program is competitive, and the number of students is limited by the number of faculty and clinical facilities available. Meeting minimum requirements does not guarantee acceptance. NOTE: THE NURSING EDUCATION PROGRAM IS CURRENTLY UNDERGOING REVISION.
