You are being invited to take part in a research study to evaluate (insert topic). The reason
we have contacted you is because (insert reason). Before you decide it is important for you
to understand why the research is being done and what it will involve. Please take time to
read the following information carefully. Please ask (insert researchers name) if there is
anything you do not understand or of you would like further information.
This study is designed to find out (insert question of research)
You will be asked to carry out (specify what and for how long). For example, the
assessments will ask you to provide details on (insert content of questions). We will send a
questionnaire for you to fill out at your convenience. All information obtained during the
study will be confidential.
We hope that you feel able to help us with this study. If at any time you decide that you do
not want to continue to take part in the study, you are free to withdraw.

You are invited to take part in a questionnaire survey exploring (insert topic).
The aim of the study is (insert aim). Questions will be asked about (insert content of
questions). I will ask you to complete this questionnaire again (insert time) to see whether
or how your responses have changed.
Participation in this study is totally voluntary, and you are under no obligation to take part in
this study. You are free to withdraw at any point prior to returning the questionnaire (you
may destroy the questionnaire of you wish). All data collected will be kept confidential and
used for research purposes only. The consent forms will be kept separate from the
questionnaires. Please briefly examine the questionnaire before signing this consent form.
There are no trick questions in this survey. There are no right or wrong answers. Your
name or any identifying characteristics will not be available to anyone, other than my
supervisor and me, at any point.
