The candidate must submit the original and 4 copies of this form (signed by the candidate and all members of the
Thesis Committee) to the Graduate Program Director at least 4 weeks prior to the examination, or oral defense of the
thesis. The defense occurs at the site of the exhibition with full graduate faculty members in attendance. Both the
thesis exhibition and the accompanying written thesis are the focus of this comprehensive review. The candidate
must meet with the Thesis Committee for guidance and recommendations prior to the oral defense.
