After that date you may register in-person only from September 1 through September 15, 2015 based on course availability. You will be required to register using a Registration Pin Number issued by your advisor. Please note: new students will fill out a registration card for their first semester. Here’s how it works: 1. Contact your advisor to discuss your course selections. This is mandatory since the Registration Pin Number will serve as your advisor’s signature and approval. 2. Advisor approves your course selections and issues a Registration Pin Number. Once you have completed the above steps, you are now ready to register via the web. Payment is due in FULL by the first day of classes. ** If arrangements are not made to pay your bill, you may be deregistered as a result**. Students can elect to use the deferred payment plan to pay their tuition. Please see payment policy for details. If payment is not received by the due date, you will be assessed a $50 late fee. Page 3 Variable Credit Courses When registering for a variable credit course like Fieldwork II 1-5 credits, you must manually enter the number of credits in the credit column located on the registration screen. If you do not enter number of credits, the system will default to 1.00 credit of Fieldwork II. The same applies for any other variable credit course. Students who are non-compliant will be unable to register for classes. Late Registration after Classes Begin September 1 through September 15, 2015 is late registration for all students with a $50 late fee. Late registration must be done in person through the Office of the Registrar or for Newark students please go to the campus office. Registration cards are available either in your campus office or the Office of the Registrar. A drop box is available for registration after 4:30 pm at the Office of the Registrar and at the Newark Campus Office. Confirmation of Registration You can view your course selections via Web for Student How Do I Add/Drop a Course? Adjustments to your schedule can be made via the web at http://sph.rutgers.edu/academics/registration/register.html from April 6, 2015, 2014 through August 31, st 2015. All drops after August 31 must be conducted in person through your campus office or the Office of the Registrar. What If I Need to Take a Leave? If you do not plan to enroll for the Fall 2015 semester, a Notification of Absence form must be submitted to your campus office before August 31, 2015. Forms are available in your campus office or the Office of the Registrar or online at http://sph.rutgers.edu/academics/registration/forms.html Students who are not planning to register for one or more semesters and do not plan to withdraw from the school must petition for an official “Notification of Absence” with the campus office during the registration period that the leave of absence will become effective. This policy applies to students enrolled in a dual degree program with another Rutgers School or another university who, because of the requirements of the dual degree program, are not registering at the Rutgers School of Public Health for one or more semesters. When Do I Register for Fieldwork? Fieldwork I (FDWC I): Student should complete their Department specific form. Once completed obtain a signature OR attach an email from your Academic Advisor or Fieldwork Faculty Advisor issuing permission to register. Students should give the approved form to the Fieldwork Coordinator for processing. Fieldwork II (FDWCII): Page 5 Meet with the Fieldwork Coordinator and the Fieldwork Faculty Advisor to discuss fieldwork options and progress. When registering for a variable cerdit course like FDWC II (1 to 5 credits), students must manually enter the number of credits in the credit column located on the registration screen. If this is not done, the system will default to one (1) credit of FDWC II. All students taking Fieldwork II are required to contact the Fieldwork Coordinator for their specific campus before registering to ensure that it is appropriate for you to register for this. When a student has registered for Fieldwork I (1 credit) and Fieldwork II (all 5 credits) and has not finished the project, the student must register for Fieldwork II Continuation. This takes into consideration that some students may need an additional semester to wrap up the project. Fieldwork II Continuation is a non-credit course and carries a modest fee of $35.00. Students are permitted to register for Fieldwork II Continuation only once. However, if a student requires additional time, after Fieldwork II Continuation, the student must enroll the following semester in Fieldwork III Continuation (and subsequent semesters, if necessary, until the fieldwork project is finished). A one-credit fee will be assessed for each additional consecutive semester of Fieldwork III Continuation at the usual tuition and fee rate. How Do I Register for Independent Study? Students interested in an independent study must complete the Independent Study Form and have it approved by an advisor to ensure that the project can be accepted as an elective. Form are available at all campus offices. The instructor must request a CRN number from the Registrar’s office by calling (732) 2359724. Is Health Insurance Available? Please note: It is University policy that each student enrolled in a program at the university on a full-time matriculated basis (9 credits or more) must have adequate health and accident insurance coverage. Part-time School of Public Health students who participate in clinical experience as part of their educational programs are also required to be covered by health and accident insurance. ALL STUDENTS REGISTERED FULL TIME WILL BE BILLED FOR HEALTH INSURANCE AUTOMATICALLY AND BE SENT AN INSURANCE PACKET. If you currently have another health insurance plan, the on-line Waiver form must be completed by September 25, 2015 to avoid being automatically billed for the Student Health Insurance Plan or being non-compliant with Rutgers University policy. Students who do not complete the insurance waiver by form September 25, 2015 will be charged for health insurance. Log on to http://firststudent.com/ Select: Rutgers Biomedical and Health Sciences, under Find you School Go to Customer Service Center Select the Waiver Your School’s Health Insurance from the menu on the left side of the page. Complete the form. Be sure to accurately enter all required information. If you do not complete all required information, the form will not be accepted. Submit the Form. You will receive a confirmation that your Waiver Form was successfully completed. Page 6 NOTE: If an SPH student drops within the add/drop period and becomes less-than full time, health insurance will be automatically dropped by student accounts. Student must contact the Student Accounts office immediately at (732) 235-9174 and inform them of the situation. The Student Accounts Office will work with students to resolve the problem. Students who drop down to part-time status but want to maintain their insurance coverage must contact their campus office and the insurance representative immediately. The University, in conjunction with University Health Care, offers a high-quality plan which fulfills the Rutgers insurance requirements. Enrollment forms are located in the campus office or on-line at www.universityhealthplans.com. Planning to Graduate? Students must submit a graduation application along with the $40 application fee. The form can be obtained through your campus office or online at: http://sph.rutgers.edu/academics/registration/docs/ApplicationDiplomaCertificate.pdf Students earning degrees after the conclusion of the Spring semester are eligible to participate in Convocation and Commencement exercises MAY 2016. The timely filing of the application is the sole responsibility of the student. The following are the deadlines for submission of graduation applications: Graduating in: Application due: January October 1 May January 2 October June 1 VITAL INFORMATION: Candidates for graduation must also: make an appointment for their financial aid (if applicable) exit interview at least one month prior to graduation; receive clearance for all materials borrowed from the libraries; and return student identification cards to the campus administrative office or Public Safety. Biostatistics Quantitative Skill Assessment Students must take and receive a passing score on the QSA before registering for the core course, Introduction to Biostatistics (PHCO 0504). Based upon performance on the QSA, each student will be advised if he or she needs additional preparation prior to registering for the course. Students admitted into Master of Science or PhD and DrPH Degree program in the Department of Biostatistics are exempt from taking the test. A. Attempts - If a student does not pass the QSA on the first attempt, they are allowed a second attempt which must be scheduled at least one week later. If a third attempt is required, it must be scheduled at least one month after the second attempt. If a student passes the QSA before the first meeting of the course “Introduction to Biostatistics” (PHCO 0504) they may register for the course, otherwise they will need to enroll in that course in a subsequent semester. B. Failure to Pass Quantitative Skills Assessment - Students who fail to pass the QSA after three attempts will be required to take and pass an entry-level college algebra course with a grade of ‘B’ or better before registering for the core course, Introduction to Biostatistics (PHCO 0504) Change of Address or Phone Number Please request a student correction form for any changes regarding name, address and phone numbers. ALL NAME CHANGES MUST INCLUDE DOCUMENTATION OF LEGAL CHANGE and the Change of Name form must be notarized before any changes can be made. Students may sign up online for Tuition Payment Plan after they have received their semester bill which will go to your school e-mail account. 2. The link is https://tuition.scholarchip.com. 3. Students with accounts in arrears who have established a payment plan and are timely in their payments shall be permitted to register for the subsequent semester. Students, who have established a payment plan but have missed two or more payments, shall immediately be placed on administrative suspension by the Business Office until the balance is paid in full. Such students will be notified of this status by the Associate Controller, Business Office. Students who do not resolve their account after two notifications from the Business Office will not be permitted to continue to attend classes during the semester in which they are currently enrolled. If account has an outstanding balance after the final due date, a late fee of $50.00 will be charged to your account each month until the balance is paid. Any student who elects to utilize the installment payment plan and finds that he or she is unable to meet this obligation must contact the Bursar’s Office immediately to arrange to fulfill his/her obligation. Lacking such arrangements, they will not be permitted to register for the following semester until all previous financial obligations are fulfilled. No degree will be awarded to a student until all financial accounts are fulfilled to the satisfaction of the University. How Do I Apply for Financial Aid? 2015-2016 FINANCIAL AID PROCESS 1. Complete the FAFSA (www.fafsa.ed.gov) using the school code: 013645 or 002629. If you are required to file federal taxes the Student Financial Aid Office strongly recommends that students and/or parents file their federal income tax returns as soon as possible and then allow 2 weeks for the IRS to process your return. After your taxes have been processed by the IRS , complete the FAFSA using the IRS Data Retrieval Tool (DRT). The IRS Data Retrieval Process simplifies and expedites the FAFSA process. 2. Complete the 2015-2016 Rutgers Financial Aid Application located in the www.myportal.rutgers.edu 3. When your FAFSA is completed (allow 7 business days for processing) you can review your 2015-2016 financial aid tracking requirements via the www.myportal.rutgers.edu 4. Do not send any additional paperwork that is not listed on your tracking requirements. If you have any questions or concerns, please do not hesitate to contact the New Brunswick Financial Aid Office. WEB FOR STUDENT INFORMATION SYSTEM - An electronic campus-wide information system, and is available to all students, faculty and staff through the Academic Computing Services (ACS) network. All students are expected to have an account with ACS. This website contains information on University policies and procedures, curriculum at other Rutgers schools, housing information, etc. For information about contacting ACS, please consult the Academic Computing Center. Page 8 General Banner Web Instructions for Students: The School of Public Health uses BANNER Web for Student Information System allows all current students to view their academic and financial records, as well as Schools’ course schedules. Students in certain programs are also able to register for courses online. The website is available through links on http://www.myportal.rutgers.edu or http://sph.rutgers.edu Before You Log On: There are a few things you need to know to effectively use the Web for Student Information System. The website is divided into two sections, a public area, which can be accessed by anyone. It includes Course Schedules and General Student Financial Aid information. The Course Schedules will allow you to access course information offered by each School for each term. The General Student Financial Aid area will link you to the Student Financial Aid website. The second section of the website is a secured area which can only be accessed by current students and requires the entry of your Student ID and PIN. This ID and PIN should not be shared with others. In the Student Records area, students will be able to view their course grades and view and print an unofficial copy of a web version of their academic transcript. A view of their account is summarized by term and cumulatively. Student Tax Information (IRS Form 1098-T) is also available. The Personal Information area allows students to change a PIN and view addresses and other personal data. All e-mail communications will be sent to your Academic Computing Account e-mail address. After successfully logging onto the Web for Student Information System for the first time, you will be prompted to change your PIN number. All PINs are automatically set as expired in order to prompt you to change your PIN to a number that only you will know. You will also be able to provide a question and answer hint to enable the Service Center to assist you if you forget your PIN. 2. Follow the on-screen instructions for changing your PIN. A Pin must be 6 digits long. You cannot use any letters or other symbols in your PIN. 3. If you fail 5 times to login correctly, web access to your student records will be denied. In order to reactivate your web access, you will need to contact the Service Center at (732) 743-3200. 4. There is a 30-minute session inactivity timeout. This means that if you do not use the system for 30 minutes while logged onto your student record, you will receive an inactivity time-out message and you will be prompted to re-enter your student ID and PIN number to regain access to the system. This timeout is to help ensure the security and privacy of your student records. Important Information Regarding the Use of Cookies The Web for Student Information System requires the use of cookies to verify authorization information. Cookies are short pieces of data used by web servers to help identify web users. These cookies are only used while you are logged onto the Web. These cookies are NOT retained by your computer after properly exiting the system and closing your browser. Logging Off the Web Use the “Exit” button at the top right of the screen to end your session so that no one will be able to access your records without re-entering your student ID and PIN. Page 9 FERPA Rutgers, The State University of New Jersey ("Rutgers") complies with the Family Educational Rights and Privacy Act ("FERPA") and makes public announcement of the law. This announcement shall be published as an "official notice" at least twice per year and will also be archived on the Office of Student Affairs Compliance Website. Students with questions about this notice or about FERPA generally should contact Jackie Moran (rucomply@rutgers.edu, (848) 932-8576). FERPA affords students certain rights with respect to their "education records" as that term is defined in the law. These rights include the following: 1. The student has the right to inspect and review his/her education records within 45 days of the date Rutgers receives a proper request for access to such records. 2. The student has the right to request amendment of education records that the student believes are inaccurate or misleading. 3. Rutgers shall obtain the prior consent of the student before disclosing personally identifiable information contained in the student's education records, EXCEPT to the extent FERPA authorizes disclosure without consent. 4. The student may direct complaints concerning the alleged failure of Rutgers to comply with the requirements of FERPA to the Office of Student Affairs Compliance, 83 Somerset Street, New Brunswick, New Jersey 08901, (848) 932-8576, or to the U.S. Department of Education, c/o Family Policy Compliance Office, U. S. Department of Education, 400 Maryland Ave., SW, Washington, DC, 20202-4605. Inspection and Review: Students wishing to exercise their rights to inspect and review their education records should submit a written request to the appropriate official at the Rutgers office which is custodian of the records they wish to review. The Rutgers official will make arrangement for access and notify the student of the time and place where the records may be inspected. Students who do not know which Rutgers office is the custodian of the records they wish to inspect should contact their Dean of Students or the Office of Student Affairs Compliance. Amendment of Records: Students requesting amendment of education records should first review the policies and procedures of their college or school and/or consult with their Dean of Students, or equivalent official. With regard to education records maintained at a Rutgers office other than one within the student's college or school, the student should write the Rutgers official responsible for the record, identify the part of the record the student wants changed and specify why the student believes the record is inaccurate or misleading. If the record custodian denies the request to amend the record, the student will be notified of the decision and advised of his/her right to a formal hearing. A student may obtain additional information on the hearing procedure from the Office of Student Affairs Compliance after an amendment request has been denied. Disclosure of Student Information without Prior Consent: There are a number of exceptions to the general rule prohibiting disclosure of personally identifiable information from education records without prior consent of the student. More information concerning these exceptions is available from the Office of Student Affairs Compliance. Some common exceptions used by Rutgers are set forth below; however there are other exceptions where education records are released without prior consent not listed here. 1. FERPA permits disclosure to Rutgers officials with legitimate educational interest in the records being sought. A Rutgers official is a person employed by Rutgers in an administrative, supervisory, academic, research or support staff position (including law enforcement unit personnel and health staff); a person or company with whom the University has contracted (such as an attorney, auditor, or collection agent); a person serving on the Board of Governors or Board of Trustees, a student serving on an official committee such as a disciplinary hearing board, or a student who is assisting another school official in performing his or her job responsibilities. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her job responsibilities. 2. FERPA allows the University to release student "directory information" without a student's consent, unless the student has requested such information be kept confidential. There is no brick and mortar bookstore on the Newark Campus. The UMDNJ Legacy Libraries web page is designed to assist students in locating and buying required text books and other course materials online. The Rutgers University Newark Campus Bookstore will also take textbook orders from SPH students for Core, URHA, QNME and DNPH courses. All vehicles owned, operated or parked on the University campus must be registered with the Public Safety Office. Parking tags may be purchased at the Office of Public Safety, Administrative Complex, Building 5, Room 531, 30 Bergen Street, Newark, New Jersey, (973) 982-5489. Office hours are 7:30 am – 3:00 pm. RU and NJIT students taking classes on the Newark Campus MUST complete a Parking Exchange Form in the campus office for parking privileges. Information regarding the parking program can also be accessed online. How Do I Get An E-mail Account? Official Rutgers email is our means of communicating with you. Faculty and staff will use email to inform you of cancelled courses, class room changes, etc.) The Academic Computing Center provides students with free university e-mail accounts and off-the-shelf applications for word processing, worksheets, presentation tools, research and publication graphics, statistics, and information access. It is located in Medical Science Building Level C, Room C630. It is the student’s responsibility to activate and frequently monitor their Rutgers e-mail accounts for time sensitive announcements, such as weather updates and canceled courses. Completed forms can be dropped off at the Newark Campus Office, 65 Bergen Street, SSB 701, Newark, NJ 07107. For your convenience, there is also drop box located on the wall outside of the campus office. Students with completed forms with appropriate signatures may place the forms in the box to be processed. Please be sure to contact the campus office to confirm receipt of your form. The lounge is open to students 24 hours a day, 7 days a week. Students must access the lounge by swipe or reader access of their student identification cards. Is there a Computer Lab? Page 13 The computer lab has limited hours and is located on the 7th floor in Room 751 in the Stanley S. Bergen Building (SSB) in Newark. YOU MUST USE YOUR STUDENT I.D. CARD TO GAIN ENTRANCE. This computing lab is not open on weekends. Computer programs in this lab include SAS, SPSS and Epi Info. There is also an Academic Computing Services Lab (ACS) located on C-Level in the Medical Science Building (MSB) in room C632. This computing lab is open in the evenings and on weekends. Student Identification Students are required to have their student identification card on them at all times on campus. Building and parking deck access after 6:00pm requires scanning of the student identification card. Is There Somewhere To Have Dinner On Campus? The University Hospital (UH) Cafeteria, The Garden Café, is located on B-level of the hospital. Hours of operation are 6:30am - 7pm. A Burger King restaurant is also located in the cafeteria and vending machines are available 24-hours a day in the (UH) cafeteria and on the ground floor of the SSB. Cafeteria service is also available during the day at the "Balcony Café" locate on the 2nd floor of the Doctor's Office Center (DOC). Is There Child Care Available? KinderCare Learning Centers offer childcare to children of Rutgers employees and students on the Newark and New Brunswickcampuses. I am Having Problems with My Identification Card, or I Have Lost It If you are having problems with your I.D. card or have lost it, please contact your campus Public Safety office at 55 Commercial Ave, 3rd floor, downtown New Brunswick. Your Rutgers ID card allows you access to Robert Wood Johnson Medical School and Liberty Plaza facilities as well as the SPH Building. Is There Security on Campus and Do I Need an Identification Card? The doors leading into the SPH building lock at 6:00pm. What this means to you is that upon your arrival for a 6:10pm class, you will find all entrances locked. However, access to any entrance after 6:00pm is possible by using your identification card. The back of your I.D. card has a magnetic strip (similar to an ATM card) and once activated/encoded works like a door to an ATM machine. To obtain an I.D., card you will need to bring photo identification to the Public Safety Office. Where is the School of Public Health computer lab and what is hours of operation? As a School of Public Health student, you have been given an account entitling you to full access. The Academic Computer Services (ACS) provides these services. The Academic Computing Lab is located on the 2nd floor in room L201 in the RWJMS building in New Brunswick). Please go to the ACS lab and activate your account.
