1. MEMBER PRIVILEGES
1.1 Free Frame – Changing your look could never get easier! Choose the frame
you like and you are entitled to free frame on your next purchase within one
year and you only need to pay for the lenses.
a) Member is entitled to free frame of the same value purchased (or less) for
the package of RM138 / RM168 / RM198 / RM238 and member only need
to pay for the price of prescription lens. Prices of lens are subject to
change without prior notice.
b) The entitlement for Free Frame Purchase is valid for twelve (12) months
from the date of last purchase and shall continue to be valid as long as
the member makes the Free Frame Purchase within the twelve (12)
months period from the member’s last / latest purchase.
c) There is no membership expiry date. However the privilege of Free Frame
will expire if the privilege is not exercise over twelve (12) months from date
of last purchase.
d) A-Card must be presented at the time of purchase.
e) Not applicable for products other than the packages under 1.1 (a).
1.2 Special Rate – So your vision has changed or the lenses have been scratched,
and you need new lenses for your frame. No worries, with every lens upgrade
you will be entitled to 10% discount from the price of lens. The discount is not
applicable for price of lens of the package under 1.1 (a).
1.3 Family Special – Bring your parents (55 years old and above, IC to be shown)
to any of our A-Look outlets and we will offer them a 50% discount on each of
their purchase. We are sure they want to look good too! The discount only
applies to purchase of the package under 1.1 (a).
2. TERMS & CONDITIONS
Use of A-Card Membership (A-Card) constitutes acceptance of the following
terms and conditions by each A-Card Member.
2.1 General Terms & Conditions for Application
a) By applying or using the A-Card Membership, the Member agrees to be
bound by the membership terms & conditions, and A-Look Eyewear Sdn.
Bhd. reserved the right to amend these terms and conditions as deemed
fit at their discretion and without prior notice.
b) A-Card is a privilege card. It is not a credit card, debit card, charge card
or discount card.
c) A-Card in non-transferable.
2.2 Qualifying Purchase Requirement
a) Customer applying for A-Card Membership must conduct a qualifying
purchase of a prescription spectacle (frame + lenses package) is required
to join A-Card Membership Program.
b) Qualifying purchase of frame + lenses package applies for A-Card
Membership with price categories as per below-listed:
RM138.00 – A-Stylish
RM168.00 – A-Fantastic
RM198.00 – A-Glamour
RM238.00 – A-Exclusive
c) An administration fee of RM15.00 will be charged.
d) A-Card Membership Program is open to all eligible Malaysian and
Singaporean individuals.
2.3 Replacement of A-Card
a) A-Card Member may obtain a replacement A-Card by returning the
damaged A-Card to any A-Look outlet or requesting for replacement of
lost card.
b) A fee of RM15.00 is chargeable for any replacement of lost or damaged
cards.
2.4 Disclaimers: Limitation of Liability
a) A-Look Eyewear Sdn. Bhd. reserves the right the change the privilege and
promotion at anytime without obligation, and reserves the right to
suspend or terminate the program at anytime without prior notice and will
not on any account whatsoever be liable for the suspension or termination
of the program.
b) In case of dispute, the decision of A-Look Eyewear Sdn. Bhd. shall be final
and binding.
I hereby declare that, I wish to apply for A-Card Membership and the above information is true and
accurate. I have read the terms and conditions as printed on this form and agree to be bound by the
same together with any and all amendments made thereto. I further agree that I am not required to
receive any notification of such whatsoever amendments.
